-Tex Pumping, LLC pumped 750sk of lead cement of 50/50 POZ with Class H, 13.5ppg, 1.51 yield, 5% KCL, 4% Gel, 0.5% FI-17, 0.1% C-20, and 0.2% X-Air. O-Tex pumped 1120sk of Class H Tail cement, 15.3ppg, 1.27 yield, 5% KCL, 0.2% X-Air, 0.6% C-12, 0.1% C-51, 0.1% C-35, and 0.05% C-20. O-Tex displaced cement with 554bbl of 11.0ppg Oil Based Mud. Circulation was lost at 490bbl into displacement. Plug was bumped, floats held, and no spacer or cement to surface. I informed American Energy Utica that a Cement Bond Log would be required to evaluate the tops of cement. While I was on site for the cement job one of O--visit the site during the daylight to ensure that the area is properly cleaned up.

0575879850 2015-05-20 Drill / Deepen / Reopen No Violations On the date of inspection I found that Warrior Services was on site in the process of running a Cement Bond Log across the 9.625 inch casing that was cemented the previous night.
0601858001 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of natural reservoir pressure and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:
6- 500 barrel production tanks

1- Combustor 2- Compressors 3- Wells producing off of natural reservoir pressure 3- Gas Production units, equipped with internal heaters and sand traps. 1- Heater treater unit 1- Low pressure separator

The site passed preliminary restoration in accordance with ORC 1509.072 A.
0786543547 2017-04-12 Production Wells No Violations On the day of my inspection accompanied by Inspector Randy Akins I witnessed metal containment has six 500 barrel tanks, four are condensate and two are produced water. Production equipment including two flares, two compressors, two heater treaters, and three seperators on site. Well is equipped with christmas tree type welhead with open valves. Gauges show no surface casing annular and 270 psi on intermediate annular.
1427651293 2015-03-29 Casing String Witnessed No Violations Upon arrival on location I found that the crew had drilled to the depth of 100 feet an36 inch hole it insert a 30 inch casing for the conductor casing. Universal will be cementing the casing to surface. Established circulation and circulated 16 bbls of good cement returns to surface.
1431429713 2015-05-11 Casing String Witnessed No Violations upon arrival on location I found the crew installing 18 5/8 inch surface casing to the depth of 400 feet. O-Tex will do the cementing. Established circulation and circulated 51 bbls of good cement back to surface.
1728301081 2018-08-06 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad containing three properly labeled producing wells. Surface and intermediate annular pressure is 0. Production equipment includes six 500 barrel steel tanks within their own containment, three vertical seperators, three gas production units, one line heater, a vapor recovery system, a combustor, and a compressor. Pad is gated and locked. Outslopes are vegetated. Site meets requirements of OAC 1509.072A for preliminary restoration.
