1141001861 2015-06-03 Casing String Witnessed No Violations I was on the Denoon #4H to witness the placement and cementing of the 13.375 surface casing. Nomac Drilling Rig # 73 drilled a 17.5 bore hole to 605 KB. Wyoming Casing Service ran 15 joints of 13.375, 54.5 lb./ft., BT&C casing, six (6) bowspring centralizers, one (1) float shoe and one (1) float collar to 602. Schlumberger cemented the casing with 550 sacks of Class A cement.Mix water temperature was 63 F and pH - 7. Cement slurry was verified by scale ( 15.3 ppg, 15.5 ppg ). Cement returns were observed at 56 barrels into the displacement ( 30 barrels returned to the cellar @ 15.4 ppg ). A top plug was landed at 206 psi and increased to 768 psi. The pressure was held for five ( 5 ) minutes without any pressure decline. Upon releasing the pressure .5 barrel flowed back and stopped. All floats held. No violations were observed.
1347676668 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

9 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

1424618505 2015-02-22 Casing String Witnessed No Violations Upon arrival on location I found Express energy installing 20 casing for conductor pipe. They are setting it at 120 feet and cementing to surface. Established circulation and circulated 5 bbls of good cement to surface.
1819959916 2015-06-16 Casing String Witnessed No Violations I was on the Denoon # 4H location to witness the placement and cementing of the 5.5 production casing. Wyoming Casing Service ran 17,321 of 5.5 casing into the bore hole. Schlumberger mixed 60 barrels of MUDPUSH ( 12.5 ppg ) to condition the hole. The spacer was followed by the dropping of the bottom plug and 293 barrels of lead cement ( 13.5 ppg ) . After the lead cement slurry Schlumberger mixed 473 barrels of tail cement ( 15.6 ppg ) slurry and displaced both slurries with a top plug and 20 barrels of sugar water and 380 barrels of treated water. All cement weghts were verified by scale. The displacement plug was landed at 2435 psi and increased to 3005 psi. This final pressure was held for five (5) minutes without any pressure decline. The pressure was bled off and three (3) barrels flowed back. All floats held. Then Schlumberger pressured upon the casing to rupture the pressure disc and pumped approximately five (5) barrels of sugar water to create a wet shoe . No violations were identified during this inspection.
