0587273005 2015-04-29 Casing String Witnessed No Violations I was on the Denoon # 1 location to witness the placement and cementing of the 13.375 surface casing. Nomac Drilling Rig # 73 drilled a 17.5 borehole from 120KB to 560 KB. Wyoming Casing Service ran 13 joints of 13.375, 54.5 lb/ft, BT&C, J-55 casing, float shoe, float collar, casing hanger and six (6) bowspring centralizers. Total tally was 540 set at 558 KB. Baker Hughes conditioned the borehole with 25 barrels of gelled spacer and then cemented with 462 sacks of Class A and displaced the cement slurry ( 15.5 ppg verified with scale ) with a top plug and 77 barrels of water. The plug landed at 240 psi and the pressure increased to 800 psi. The pressure was held for five (5) minutes without any pressure decline. Upon bleeding off the pressure, .75 barrel flowed back and stopped. All floats held. Cement slurry returns ( 13.8 ppg verified by scale ) were returned to the cellar with 20 barrels left in the displacement. No violations were identified during this inspection.
1410332596 2015-04-30 Casing String Witnessed No Violations I was on location to witness the placement and cementing of the 9.625 intermediate casing. Nomac Drilling Rig # 73 drilled a 12.25 borehole, on misted air, from 500 to 2515 KB. Wyoming Casing Service ran a float shoe ( 1.8 ), two (2) 9.625 casing joints ( 88.09 ), a float collar ( 1.5), 54 joints of 9.625 ( 2378.77) and 16 - bow spring centralizers. Total casing string tally was 2475. Baker Hughes conditioned the borehole with 20 barrels of water followed by 25 barrels of gelled spacer. Baker Hughes mixed and pumped 918 sacks ( 193 barrels, 15.6 ppg ) of Class A cement blended with 0.05% SF, 0.5% CaCl and 0.3% A10. The cement slurry was displaced back to the cellar with a top plug and 183 barrels of water. The plug was landed and bumped up to 1545 psi and held for five (5) minutes for the green cement test. After five minutes without any pressure decline the casing pressure was released and flowed back 0.5 barrel. All floats held. I observed circulated back to the cellar 25 barrels of cement ( 15.4 ppg ). No violations were identified during this inspection.
1424703573 2015-02-22 Casing String Witnessed No Violations The rig ran drilled 120 of 20 casing and Express Energy cemented it with 230 sacks of Class A cement. Good cement was rturned to the surface. See the casing ticket for the details.
1474319311 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

9 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

1795401564 2015-04-27 Production Wells No Violations I was on the Denoon pad to inspect the Denoon # 1 H location. Nomac Drilling Rig # 73 was completing rigging up to spud. Global Oil & Gas Services was on location to fill the SOBM tanks and Curry Services was filling the fresh water tanks. No violations were identified during this inspection.
2007350619 2015-05-04 Drill / Deepen / Reopen No Violations I inspected the Denoon # 1-H location on 5/4/2015. Nomac Drilling Rig #73 was drilling at 7544 KB on SOBM. Panhandle Tubular Inspection Service was on location conditioning the 5.5 production casing and Fairmont Tool was dressing the drill pipe. Inspections of the cellar, mud tanks and solids handling area did not reveal any violations at this time.
