0112957374 2015-02-21 Casing String Witnessed No Violations The rig ran 9-5/ Caing to 2506. Schlumberger cemented it with 907 sacks of Class A cement. Good cement was returned to the surface. A pressure test of the casing at the end of the job fell with in the parameters set forth in the law. See the Casing Ticket for the details.
0135091108 2015-02-23 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. The rig mudded up at a depth of 7192 and is currently drilling the 8.5 curve at a total measured depth of 7423. 5.5 production casing is being delivered to the racks in front of the rig. It is new, 20lb/ft., P-110 casing. I met with the company men and discussed the issues they had during the surface casing cement job with O-Tex Pumping (see cement report). They will be using Schlumberger for the primary cement job. The location is in good condition and all drilling equipment is on containment. No violations.
0620976236 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap, a gas production unit and a line heater. Production equipment that was shared with permitted wells 34067212280000, 34067212290000, 34067212300000, 34067212310000, 34067212320000, 34067212330000, and 34067214320000 included a vapor recovery system, 5 gas compressors, a gas combustor and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1398110251 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 5 compressors

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1424303578 2015-02-18 Casing String Witnessed No Violations Nomac Rig 73 ran 14 joints of R-3 54.5 # casing to 517. O-tex Pumping cemented it with 490 sacks of Class H cement. Good cement was returned to the surface. See the casing ticket for the details.
1429624338 2015-03-06 Drill / Deepen / Reopen No Violations I was on location to inspect the drilling operations of Nomac Rig # 73. They have drilled the 8.5 lateral to a total measured depth of 18,057 and are currently circulating to clean up the borehole. They will begin tripping the drill pipe out of the hole this afternoon and rig up to run 5.5 production casing. I inspected the casing and it is new, 20lb/ft., Grade P-110 casing. All drilling equipment is on containment. No violations.
