0742949631 2016-06-22 Preliminary Restoration No Violations I arrived on location, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

4 400 bbl. Production tanks 5 Producing wells

1 Compressor 1 Vapor recovery system 1 combustor

 1 heater treater 

The site passed preliminary restoration in accordance with ORC 1509.072 A

1276413166 2016-09-07 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 2000 psi. Fluids and gases could be transferring from the wellhead to the flow line. I use unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.

Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1451929026 2016-01-04 Drill / Deepen / Reopen No Violations I arrived on location to check on the drilling status of Nabors Rig No.36. I met with Company Man John Mode. John informed me that Nabors hit the total depth for the well bore and was in the process of tripping drill pipe out of hole. The production casing was set to be run later in the day. John also informed me the cement job for the production casing will occur at approximately 7 pm on 1/5/215. While on site I visually conducted an inspection of solids control where I found everything clean and in order. There were no leaks or spills outside of containment
1676649749 2016-02-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Hess Ohio was in the process of fracking the well bores drilled on this pad. While I was on site I performed a visual inspection, were I found no signs of pollution or contamination.
1726015939 2017-12-11 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 producing wells. Wells and production equipment are on different pads. The surface casing had 0 psi and the intermediate casing had 0 psi. Wellheads were labeled. Production equipment includes five GPUs, four 400 barrel steel tanks, a compressor, a heater treater, and a flare. Production records are current. Site meets prelimary restoration requirements of ORC 1509.072A.

2005062424 2015-09-08 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. HWD Drilling Rig # 11 drilled a 17.5 inch diameter hole to 550 ft. and set 540 ft. of 54.5 lb. casing. Schlumberger Cementing Service cemented with 540 sacks of class A cement at 15.6 lb. and a yield of 1.20 and had 17 bbl. of good cement returns to surface.
