0238178182 2014-12-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Chevron has pump a plug again. The water from the underground stream has not been shut off. The rig shut down waiting on the cement to cure. When I arrived on location they were just starting to run the drill pipe back into the wellbore to tag the cement. Tagged top of cement at 265 feet.
0487382303 2015-02-24 Drill / Deepen / Reopen No Violations Upon arrival on location I found the crew preparing to start spudding the curve section of the well. They still had about 1.5 hours left on the wait time on the cement that the pumped for the kick off point.

1418152421 2014-12-09 Plug / Plug Back No Violations On the day of my inspection I found the rig crew running drill pip in the wellbore. A cement plug was set from 320 feet to 420 feet to try any shut of the water of an underground stream. When the crew drill through this section it was unable to handle the amount of water the well was making. It was decided to set the plug in an attempt to shut off the water. After running in with the drill pipe the top of the cement was tagged at 385 feet. It was determined that the water was not shut off. Chevron decided to set another plug in trying to shut off the water.
1422470493 2015-01-28 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nabors Rig X07 has moved on location to complete the drilling operation. I met with Russell with Chevron. They have not started drilling yet. They have to remove the bridge plug set in the wellbore, then the resume drilling.
1508264129 2017-10-17 Preliminary Restoration No Violations On the day of my inspection I found the pad two contain two plunger lift wells. No pressure or surface or intermediate casings. Production equipment includes two gpss, condensate heater, seven 400 barrel steel production tanks, and a vapor recovery system. Wellhead is mislabeled as permit 1386. Operator Jimmy Steele was notified. Tanks are in steel containment. Site passes preliminary restoration per ORC 1509.072A.
2102559985 2014-12-08 Plug / Plug Back No Violations I received a phone call from Dave with Chevron stating that they hit an under ground stream and could not handle that much water. They wanted to set a cement plug and try to shut the water off. They hit water at 385 feet. I ask Mark Moreland to accompany me to the location. It was agreed that a cement plug would be set at 420 feet to 320 feet, which would bring the cement into the bottom of the 13 3/8 inch casing. The plug was set with 113 sacks of cement.
