0240849716 2016-09-07 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 2400 psi. Fluids and gases could be transferring from the wellhead to the flow line. I use unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.

Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. 5- Producing wells utilizing natural reservoir pressure. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1447766793 2015-11-17 Drill / Deepen / Reopen No Violations I received a call from Inspector Mark Moreland who was called and informed that an oil/brine spill had occurred around the pump jack due to a suspected stuffing box failure. Prior to my arrival I called the Ohio Emergency Response line, to inform them of what I had just been informed. When I arrived on location Chris Belch ( Atlas Production Foreman) was on scene. He confirmed to me that the spill had occurred due the stuffing box rubber failure. Chris estimated that appromixmently 20 gallons of oil/brine had been released due this failure. Prior to my arrival Chris had placed peat sorb and oil absorbent pads down to contain the spill. The effected area from this spill measured 10 foot wide by 10 foot long, and did not leach out of its contained area or into any water source. During my inspection Atlas Energy brought in a steam powered pressure washer, and a mini excavator. The steam cleaner was used to clean of the contaminated well head and pump jack. Atlas then used the mini excavator to remove the contaminated soil and gravel. The containments was hauled off by Sutton Pump who took the manifest to a Kimble facility.

1 Compressor 1 Vapor recovery system 1 combustor

The site passed preliminary restoration in accordance with ORC 1509.072 A

1758317875 2017-12-11 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 producing wells. Wells and production equipment are on different pads. The surface casing had 0 psi and the intermediate casing had 0 psi. Wellheads were labeled. Production equipment includes five GPUs, four 400 barrel steel tanks, a compressor, a heater treater, and a flare. Production records are current. Site meets prelimary restoration requirements of ORC 1509.072A.

1969617042 2016-08-08 Production Wells No Violations On the day of my inspection will performing a routine inspection I found no issues on site. I conducted a walk around the location and found no signs of pollution or contamination.
