0374500964 2016-05-12 Drill / Deepen / Reopen No Violations I am on site today to witness the cementing of surface casing. Patterson Rig 292 drilled a 17.5 inch diameter hole to a depth of 1347 feet then ran 1324 feet of 13.375 inch diameter 61 lb/ft. surface casing. The shoe collar was set at a depth of 1312 feet. After running the casing and iron was rigged up to the floor, the rig had an electronical malfunction and the cement job was delayed until Patterson drilling got an electrician on site to fix the issue. After the rig was fixed, O-Tex Pumping established circulation then used 1380 sacks of Class A 15.6 pound/1.2 yield cement to bring cement returns to surface. After I witnessed cement returns at surface, I witnessed the wiper plug landing and O-Tex pumping 600 pounds of pressure indicating the casing string had integrity.
0378517718 2015-05-05 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Rat Hole drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118 lb. casing. Allied Cementing Service cemented with 560 sacks of class A cement at 15.6 lb. and a yield of 1.20 During the drilling of the 36 inch hole the well collapsed at 30 ft. to 38 ft. and they used 21 yards of grout to fill the collapsed section. They waited until after it had set up then drilled it out to 100 ft. The well bore will not be uniform top to bottom and that is why they are running so much extra cement.
0538485490 2018-09-18 Production Wells No Violations On the day of my inspection I was accomapanied by Inspector Ken Fry. We identified ourselves to company man Larry Byrd. Patterson 795 is on the 1404 permit waiting to cement production casing. This is a SIMOPs pad. Annular pressures on this well are zero psi. Company man did not have anticollison reports and directional driller was not on location during the inspection.

0988462619 2019-05-08 Preliminary Restoration No Violations On the day of my inspection I am visiting pad at the request of Adam Schroeder on behal of Mr. Humpreys. Mr. Humpreys was concerned with no production on the well which was due to shutting it in to drill the 1H, 3H, 5H, and 7H wells. Email was sent to Adam Schroeder informing him the well is back in production. This well is currently producing off of natural reservoir pressure. Production equipment includes five GPUs, five sand seperators, two line heaters, one combustor, three compressors, two vapor recovery towers, and eight 500 barrel steel tanks, within their own containment. The 13.375x9.625 annular pressure was 300 psi and the 9.625x5.5 annular pressure is 1300 psi. Burst pressure on the 13.375 casing is 3090 psi. Spoke with lease operator onsite and he said he would get the annular pressure blown down. Pad is stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
1047048291 2016-05-19 Drill / Deepen / Reopen No Violations I am on site today to witness the second stage of cementing the intermediate string casing. Patterson Rig 292 drilled a 12.25 inch diameter hole to a depth of 7380 feet then ran 7370 feet of 9.625 inch diameter 40 #/ft. intermediate casing. The shoe collar was set at a depth of 7367.9 feet. The stage collar was set at a depth of 3995. For the first stage of the job after Patterson had condition the hole, O-Tex Pumping established circulation then used 630 sacks of Class A (13.5 pound, 1.5 slurry yield) lead cement consisting of 50-50 Premium H/Poz, 5% KCL, 4% Gel, and .5% FL-17, then follow the lead cement with 380 sacks of Class A (15.3, 1.26 slurry yield) tail cement. Cement was brought pass the stage collar, then O-tex pumping dropped the deviation tool to open up the ports at the stage collar Company man Bill Roberts informed me that Patterson circulated cement to surface after the ports were opened (indicating the first stage went as calculated), then procced to circulate the hole for four more hours before starting the second stage. When I arrived on site I witnessed O-Tex pumping used 50 bbl. of gel before sending 1240 sacks of Class A (15.3, 1.26 slurry yield) cement. This volume of cement was calculated to bring cement returns to surface, however I witnessed approximately 40 bbl. of gel was brought to surface then the wiper plug had landed. The 40 bbl. of gel brought to surface indicated that the cement was in the surface casing beyond 200 feet and was within 100 foot of the surface (which meant all requirements). I then placed a call to my supervisor and informed him of what had happened. Bill Roberts informed me that Ascent will run a bon log after they drill the lateral section of the well bore, to see where the cement was inside the surface casing.
1147711074 2016-06-09 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by inspector Mark Moreland, I witnessed that Patterson Rig No 292 has rigged down and moved off location. A cage has been placed over the well bore.
1155457781 2016-06-01 Drill / Deepen / Reopen No Violations Upon my arrival I found Patterson Rig No 292 in the process of running the 5.5 inch diameter production casing. With a conversion to the company man, I was informed that the cement job would occur sometime that night or in the morning. After speaking to the company man I then walked around site where I found no signs of pollution or contamination outside of containment.
1204617049 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 1 properly labeled producing well and four conductors/cellars that are not labeled. Pressures on wellhead are: surface 200 psi, intermediate 700 psi, production 1000 psi, and tubing 1500 psi. Production equipment includes one GPU, a vapor recovery tower, combustor, compressor, and four 500 barrel steel tanks. Steel tanks are in metal containment and there is a berm around pad. Outslopes of pad are stable and vegetated. Pad is fenced and locked. Site meets requirements on 1509.072A for preliminary restoration.

1464099406 2016-05-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson Rig No. 292 in the process of drilling the lateral section 8.5 inch diameter hole. Company man Bill Roberts who informed me that Patterson was at a depth of approximately 14562 feet. Bill predicted that the 5.5 inch production string would likely be run in the next 48 hours. While on site I conducted a visual inspection and found no signs of pollution or contamination.
1540993008 2018-09-18 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1405. Harrison County Inspector Dylan Pendleton was present during the inspection. During the inspection, Dylan and I met with company representative Larry Hart. At the time of my inspection well #2-1405 was temporarily abandoned to meet SIMOPS permit conditions for offset wells on the pad. The well head annular valves were plumbed above grade and were open for monitoring. The well head had been lowered to the top of the B section and a night cap installed. A steel cage was placed over top of the well head. The cellar was filled with water. No sheen or gas bubbles were observed. Patterson drilling rig # 795 was on location and was preparing to cement production casing on the Sproull 7H. The directional provider was off location and an updated anti-collision plan was not available for review. No other issues were observed during this inspection.
1599389805 2016-05-17 Drill / Deepen / Reopen No Violations I am on site today in order to pick up the cement tickets from the surface casing job that occurred last week. While on site I met with company man Bill Roberts who informed me that Patterson Rig No. 292 was five feet away from hitting total depth desired for the 9 5/8 inch diameter intermediate string casing. He stated that the intermediate job would occur within the next two days.
1668688500 2018-12-20 Production Wells No Violations On the day of my inspection I was accompanied by Geologist Tammy Richards. Halliburton frac crew is on location. This well is a simops well and is shut in and under a steel cage. Surface and annular pressure is 0.
2023033773 2018-08-21 Production Wells No Violations On the day of my inspection I found this pad containing 4 conductors and 1 drilled well. This pad is currently used as a staging area for the Sherwood pad. Ascent plans on moving patterson 795 on location to drill the 4 conductors. SIMOPS plan calls for tubing plugs in both the XN and X nipples to provide for a dual/ redundant well control barrier. The wellhead has been disconnected from the flowline and is not producing. Surface, intermediate, and tubing pressures are 0 and the production gauge is reading 200 psi. A phone call from James McGee on 8/22 indicated that Hugo Perez stated the tubing plugs were set on 8/22.
2049159654 2019-01-03 Production Wells No Violations On the day of my inspection I witnessed a Halliburton frac crew preapring to move off of location. This well was previously in production and had 2 plugs set in it for SIMOPS activities. Surface and annular gauges read 0. Well head has a steel cage around it and production tree was removed and replaced with a night cap.
