0448949814 2018-01-02 Production Wells No Violations This well is equipped with 5.5-inch diameter production casing. The well appears to be free flowing with no artificial lift technology present. The well is piped to a gas processing unit and then to a production area. The production area contains three 400-barrel production fluid holding tanks. The outside of the location has chain linked fencing installed. Production records within the RBDMS system are updated to 2017.
0904039727 2014-10-17 Casing String Witnessed No Violations On the day of my inspection I found that Yost Drilling had drilled a 28 hole to a depth of 241. They ran new 24 surface casing. Allied Oil & Gas Services broke circulation and cemented the casing to surface. The cement fell approximately 10 after they stopped pumping.
1412374723 2014-10-03 Casing String Witnessed No Violations On the date of inspection I was informed by company man Matt Miller that Precision Rathole had drilled a 36 inch diameter hole to a depth of 100 feet. Precision set 30 inch diameter 118.64lbs/ft casing on bottom. Allied Oil and Gas Services established circulation and pumped 364sk of Class A cement, 15.6ppg, 1.2 yield, 2% CaCl, 0.25lbs/sk of flake. Allied pumped 75bbl of fresh water to displace cement. Fluid level was at approximately 25-30 feet prior to pumping on. After establishing circulation with 20bbl of water ahead, circulation was lost. No cement returned to surface. Fluid briefly reached surface before falling back. Matt Miller will have the hole topped off when cementing the next conductors.
1413690186 2014-10-18 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 18-5/8 inch diameter casing string which is also the Mine String. Alpha Hunter Drilling Rig # 5 drilled a 22 inch diameter hole to 344 ft. and set 344 ft. of 87.5 lb. casing. Allied Cementing Service will cement with155 sacks of class A cement at 15.6 and a yield of 1.20 for the lead on bottom below the Mine and 275 sacks of class A cement at 15.6 and a yield of 1.20 for the tail cement to top off to surface from the basket placed at 245 ft. to surface. Used 1 inch diameter tubing to spot cement in annular space between casing and hole.
1418585103 2014-12-12 Casing String Witnessed No Violations I am on site today to witness the pressure test on the 50 ft cement section below the Mine void. This section was cemented but a test is conducted to guarantee that the cement remained in place and did not u-tube into the void. The test revealed that the cement did not remain in place. The 50 ft section is in direct communication with the Mine. The corrective action is to cement this section again and repeat the test again.

2120787213 2017-09-20 Preliminary Restoration No Violations On the day of my inspection I found the Cravat wellpad to have 4 properly labeled, producing wells. Wells are equipped with christmas tree wellheads. There is 100 psi on surface casing, 0 psi on the intermediate casing, and 3200 psi on the production casing. Production equipment includes four gas production units, two line heaters, and three 400 barrel steel brine tanks. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
