0154318353 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains five properly labeled producing wellheads. Production equipment includes nine 500 barrel steel tanks in their own polymer lined steelcontainment, five GPUs, five sand seperators, two line heaters, a compressor, and a vapor recovery unit. Surface annular pressure is 100 and intermediate annular pressure is 00. Production records are current in RBDMS.
0191028281 2015-03-19 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. Patterson Drilling Rig # 292 drilled a 437 ft. hole and set 437 ft. of 87.5 lb. casing. Schlumberger Cementing Service cemented with 725 sacks of class H cement at 15.6 lbs. and a yield of 1.21 had casing on bottom and plugged off pulled 4 joints out and had to circulate back to bottom. Became ill and had to leave before job was completed.
0852081295 2015-01-27 Casing String Witnessed No Violations I arrived today to top off the cement on the conductor. After the cement job on 01/26/2015 it was discovered that the cement fell down several feet and will need topped of. Allied is on site to top off the conductor. The cement truck is frozen and after 3 hours it could not be fixed in the field and the job was cancelled until Wednesday at 10 AM.
1422285376 2015-01-26 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Drilling Rig # 1 drilled a 36 inch diameter hole to 100 ft and set 100 ft of 118.76 lb casing. Allied Cementing Service cemented with 316 sacks of class A cement at 15.6 and a yield of 1.20 had very slight returns.
1422458279 2015-01-28 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cement top off for the conductor pipe. Precision Rathole is on site and tagged cement at 38 feet below ground surface level. Smith Concrete ran approximately 3.5 cubic yards of ready-mix concrete to top off the hole and left 1 foot of cement in the bottom of the cellar.
1426779697 2015-03-18 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 292 has walked over to this well has begun drilling the surface hole. They were to be ready to set surface this morning. I arrived to find that they are behind schedule because they had to make a bit trip after the 24 inch diameter air hammer quit drilling. Should be ready later tonight. I did return to this site with Jason Simmerman and 5 OSU students along with Ron Trivisonno for a guided tour with AEU personnel.
1427049796 2015-03-22 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. . Patterson Drilling Rig # 292 drilled a 17.5 inch diameter hole to 1800 ft. and set 1800 ft. of 61 lb. casing. Schlumberger Cementing Service cemented with 700 sacks of class H lead cement at 15.3 lb. and a yield of 1.27 and 480 sacks of class H tail cement at `15.6 lbs. 70 barrels into the cement slurry the pump truck broke down and now the cement will have to be circulated out of the hole and another pump truck sent out. After checking the cement on site it was determined that there now is not enough cement to do the job. They will need to send out more cement, so job will need to be rescheduled. They are to call when they know what that time will be. The call came in with no time to witness the job, they forgot to call me until the job was started.
1572565410 2017-09-20 Preliminary Restoration No Violations On the day of my inspection I found the Conotton wellpad to have 5 properly labeled producing wells. Wells are equipped with christmas tree wellheads. There is 0 psi on surface casing and 0 psi on the intermediate casing. Production equipment includes five gas production units, two line heaters, three compressors, vapor recovery system, combustor, and nine 500 barrel steel tanks. Three are brine and six are condensate tanks. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
2049220970 2015-03-31 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 292 is at 7367 ft. and will begin the curve section of the well. I checked the solids control area and found it clean with the liner in place.
