0101053421 2015-01-23 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Drilling Rig # 3 drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118.76 lb. casing. Universal Well Service cemented with 310 sacks of class A cement at 15.6 lbs. and a yield of 1.20 and had cement returns to surface.
0469370720 2017-09-20 Preliminary Restoration No Violations On the day of my inspection I found the Conotton wellpad to have 5 properly labeled producing wells. Wells are equipped with christmas tree wellheads. There is 0 psi on surface casing and 100 psi on the intermediate casing. Production equipment includes five gas production units, two line heaters, three compressors, vapor recovery system, combustor, and nine 500 barrel steel tanks. Three are brine and six are condensate tanks. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
1178943112 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains five properly labeled producing wellheads. Production equipment includes nine 500 barrel steel tanks in their own polymer lined steelcontainment, five GPUs, five sand seperators, two line heaters, a compressor, and a vapor recovery unit. Surface annular pressure is 150 and intermediate annular pressure is 00. Production records are current in RBDMS.

1433115983 2015-05-31 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 9-5/8 inch diameter casing string. Patterson Drilling Rig # 292 drilled a 12-1/4 inch diameter hole to 7640 ft. and set 7624 ft. of 40 lb. casing. Schlumberger Cementing Service cemented with 475 sacks of class H cement at 11.2 lb. and a yield of 2.02 of lead cement and 1230 sacks of class H cement at 15.6 lb. and a yield of 1.20 of tail cement. Schlumberger Cementing Service had problems displacing cement after pumping 90 bbl. Away and after almost 25 minutes the rig mud pump took over and pumped the cement to within 20 bbl. Of the total displacement. At that time Schlumberger took over and landed the plug. Circulation was constant and did get some returns back to surface.
