0211942778 2015-02-24 Drill / Deepen / Reopen No Violations I inspected this drilling well on 2/25/2015. Patterson Drilling Rig # 329 rigged up over the R Alan # 7H conductor casing and went in the casing with the drilling string and tagged the conductor cement top at 86 KB. A walk around inspection showed the SOBM tanks being cleaned out and connecting the mud lines. This is the seventh ( 7) well on the pad. No violations.
0712568235 2015-02-25 Casing String Witnessed No Violations I was on the R Alan # 7 - H to witness the placement and cementing of the 13.375 surface casing. Patterson Drilling Rig # 329 went in the 20 conductor casing with the 17.5 bottom hole assembly ( BHA ) and tagged cement at 86 KB. Rig # 329 mist drilled a 17.5 borehole from 86 to 550 KB. Wyoming casing crew ran 12 joints of 13.375, 54.5 lbs./ft, BT&C casing, one (1) 6.7 casing hanger and one (1) 27 landing joint. Total tally ran was 540.39. Schlumberger ( SLB ) connected to the casing and established circulation with 5 barrels of water. SLB pumped a 25 barrel gelled spacer ( 8.7 ppg ) followed by 10 barrels of water spacer. SLB mixed and pumped downhole a tail cement slurry ( 15.9 ppg, verified by scale ). Total cement slurry volume was 107 barrels. The cement slurry was circulated back to the cellar using a top plug and 76.4 barrels of water. Cement slurry returns were found to weigh 15.4 ppg. I observed approximately 25 barrels of cement slurry returned to the cellar. The plug was bumped to 810 psi ( 570 psi over final lift pressure of 240 psi ) and held for five (5) minutes without any pressure decline. Once the pressure was released .5 barrel flowed back and stopped . All floats held. No violations were identified during a walk around tour.
1425928496 2015-03-08 Casing String Witnessed No Violations I was on the R Alan # 7H to observe the cementing of the 5.5 production casing. Wyoming casing service ran 333 joints of 5.5, 20 lb./ft casing to 14,474 KB. Schlumberger mixed and pumped 60 barrels of MUDPUSH, 306 barrels of lead cement slurry ( 13.5 ppg ) followed with 336 barrels of Tail cement slurry ( 15 ppg ). Schlumberger displaced the cement slurry with 320 barrels of treated water and a bottom and top plug system. The plug was landed at 2219 psi and increased to 2818 psi. After a five (5) minute static period without any pressure decline the pressure was increased to rupture the disc out of the top plug. After which three (3) barrels of sugar water was displaced to create a wet shoe then shut down. All floats held. No violations were identified during this inspection.
1480789154 2016-02-23 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

11 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 5 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

1960486773 2014-10-14 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Rocky Mountain Drilling drilled a 26 inch diameter hole to 120 ft. and set 120 ft. of 80 lb. casing. Universal Well Service cemented with 245 sacks of class A cement at 15.6 lb. and a yield of 1.20 and had good cement returns
