0568840210 2017-10-17 Preliminary Restoration No Violations On the day of my inspection I found the pad two contain two plunger lift wells. No pressure or surface or intermediate casings. Production equipment includes two gpss, condensate heater, seven 400 barrel steel production tanks, and a vapor recovery system. Tanks are in steel containment. Site passes preliminary restoration per ORC 1509.072A.
1277969275 2014-12-05 Casing String Witnessed No Violations On the day of my inspection I found that Highlands Drilling rig 14 had drilled a 17.5 hole to a depth of 375, where they ran new 13.375 casing. Baker Hughes attempted to cement the casing to surface with 686 sacks of cement. Good circulation was established and maintained throughout the job. Cement was not circulated to surface. Chevron was ordered to have a cement bond log ran on the well and grout the casing to surface.

Established circulation but did not see cement returns to surface. Ran a tempture log to determine the cement top.

Established circulation and circulated 24 bbls of good cement back to surface. They are shutting down today until January 2015. company personal will be on location the entire time, until they resume operations.
1423150513 2015-02-04 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nabors Rig X07 has reached a depth of 13932 feet. I completed a site inspection and found no issues or violations.

1725050351 2016-03-29 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

9 400 bbl. Production tanks 2 Producing wells 2 Sand separators

1 Low Pressure Separator 1 Combustor 1 Line Heater The site passed preliminary restoration in accordance with ORC 1509.072 A.
