0638112453 2015-02-24 Drill / Deepen / Reopen No Violations Upon arrival on location I found the rig has completed the well at a depth of 15224. Installed 5.5 inch casing and cemented. They were prepairing to start spudding on the next well.
1417441686 2014-12-01 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Top Hole Drilling Rig # 1 drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118 lb casing. Universal Well Service cemented with 328 sacks of class A cement at 15.6 lb. and a yield of 1.20 - There was 15 bbl. of good cement returns.
1418231225 2014-12-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found some activity on the pad. The liner is in place and the pad boards covers the liner. They are in the process of moving in some of the equipment. The entrance dirt work is all stabilized and vegetation is established.

Nomac Drilling Rig # 84 drilled a 17.5 inch diameter hole to 400 ft. and set 400 ft. of 54.5 lb. casing. O-Tex Cementing Service cemented with 630 sacks of class H cement at 15.6 lb. and yield of 1.20 and had 30 bbl. Of good cement returns.
1423844855 2015-02-13 Drill / Deepen / Reopen No Violations Up on arrival on location I found the crew pulling the drill pipe. They have completed their curve at a depth of 8400 feet.
1515608770 2018-01-10 Production Wells No Violations On the day of my inspection I found this pad to contain six christmas tree wellheads. Production equipment includes three line heaters, six gas production units, six vertical sand seperators, twelve 400 barrel steel tanks, two compressors, 2 vapor recovery towers, a combustor, and pipeline equipment. This well had zero pressure on the surface gauge and 75 psi on the intermediate gauge. Production records are current through 2017.
1809337829 2016-07-26 Preliminary Restoration No Violations I arrived on location, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells have not been fracked, however the production equipment has been place on location. RBDMS indicated that the well is not in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 2 NGL Tanks 6 Non-fracked wells with night caps

1 Vapor recovery system 1 combustor 1 Low pressure separator

2 heater treaters

1 Line Heater The site passed preliminary restoration in accordance with ORC 1509.072 A.

2065712427 2014-12-03 Drill / Deepen / Reopen No Violations On the day of my inspection I found the crew drilling and installing the pipe for the mouse holes, and removing the cement heads form the conductor casing. It will be a couple days before they are ready for the rig.
