0742098624 2018-01-10 Production Wells No Violations On the day of my inspection I found this pad to contain six christmas tree wellheads. Production equipment includes three line heaters, six gas production units, six vertical sand seperators, twelve 400 barrel steel tanks, two compressors, 2 vapor recovery towers, a combustor, and pipeline equipment. This well had zero pressure on the surface gauge and 100 psi on the intermediate gauge. Production records are current through 2017.
1081404586 2016-07-26 Preliminary Restoration No Violations I arrived on location, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells have not been fracked, however the production equipment has been place on location. RBDMS indicated that the well is not in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 2 NGL Tanks 6 Non-fracked wells with night caps

1 Vapor recovery system 1 combustor 1 Low pressure separator

2 heater treaters

1 Line Heater The site passed preliminary restoration in accordance with ORC 1509.072 A.

1426853865 2015-03-20 Casing String Witnessed No Violations On arrival on location I found the crew had drilled to a depth of 402 feet of 17.5 inch hole and was running 13 3/8 inch casing in to the wellbore. Cementing the casing to surface will be O-Tex. Established circulation and circulated 5 bbls of good cement returns to surface.
1738353901 2015-03-26 Drill / Deepen / Reopen No Violations On arrival on location I found the rig crew had drilled to a depth of 6401 feet and was running a bond log on the 9 5/8 casing. They will then run in the wellbore and start to build the curve.
