0673230634 2017-08-31 Preliminary Restoration No Violations On the day of my inspection I found the Cadiz A pad seperated between a production equipment pad and a wellhead pad. Four properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, four gas production units, heater treat, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.

1407844440 2014-08-10 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. HAD Drilling drilled a 22-1/2 inch diameter hole to 330 ft. and set 322 ft. of casing. Halliburton Energy Service cemented with 350 sacks of class A cement at 15.6 lb and a yield of 1.20 and had 19 bbls of good cement returns.
1421851678 2015-01-21 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 9-5/8 inch diameter casings. H&P Drilling Rig # 287 drilled a 12-1/4 inch diameter hole to 7650 ft. and set 7640 ft. of 40 lb. casing. Halliburton Energy Service cemented with 990 sacks of class A cement as a lead cement at 12 lb. and a yield of 2.55 followed with a tail of 320 sacks of class A cement at 13.5 lb. and a yield of 1.55 and had 50 bbl. Of cement returns.
