1402439266 2014-06-10 Casing String Witnessed No Violations Upon arrival to the location Alphia-Hunter rig #9 was running 9 5/8 inch diameter casing to the depth of 1598 feet. Halliburton Energy will be cementing to surface. At the northwest side of the location a slip has started to develop. It covers an area of 68 feet wide and 32 feet long. It has slipped about 30 inches. The Company personal has been made aware of it and an engineer and the proper personal will be on location 6/11/2014 to address the problem.

Circulation was established and circulated 15 bbl of good cement returns to surface.
1412092699 2014-09-30 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by company man Brandon Bramlage that the Nomac 245 rig had drilled an 8.75 inch diameter hole to a total depth of 17, 408 feet. At the time of inspection Wyoming Casing Crew was setting the 5.5 inch production casing and cementing would occur late tonight or early the next morning. After cementing the rig was being released.

-43 was issued on March 4, 2016 for the suspension of all operations. During my inspection accompanied by Inspector Mark Moreland I witnessed the well was in operation. At the well head I found all production valves were in the open position and pressure gauges showed readings of approximately 550 psi. After inspecting the well head I then went to the GPU (Gas Production Unit) were I discovered the unit was in operation. Sounds of fluid/gases could be heard running throughout the unit, the pressure gauges on the inlet side of the unit read approximately 550 psi and heat could be felt coming through the line heater. After verifying the well was in operation, Mark placed a called to the Emergency Number listed on the production tank batteries (866) 590-

1668205925 2016-12-15 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road in good repair and the gates to access the location were locked. With my arrival on location I witnesses I witnessed all wells on site are now in producing order. The permitted well bore is utilizing natural reservoir pressure for means of production. The production equipment that is on site consists of the following:

2- 500 barrel production tanks 7- Permitted well bores utilizing natural reservoir pressure 7- Gas production units equipped with internal heaters and sand separators 1- Low pressure separator system

The site passed preliminary restoration in accordance with ORC 1509.072 A.

In talking to a company repsentive of Eclipse the wells are still in the flow back mode and will be turned into the pipeline system to start production in two days.
