0073472377 2015-06-16 Production Wells No Violations I am on site today to find out the status of the well after it was reported to me that there was a problem with the 5-1/2 inch diameter production casing at 4950 ft. AEU reported that they were on stage 26 -27 of the frac when a casing integrity issue was discovered. The work over rig could not get tools past a section of the well at 4700 ft. to 4950 ft. There was an attempt to bleed the 3,200 psi pressure off of the well and it was discovered that the pressure after 20 minutes remained. A plug was set at 8,100 ft. and another at 6,100 ft. They tried to pump fluid at this time and it was determined that it was going into the formation at that depth and not returning up the annulus. The top of the cement on the 5-1/2 inch diameter casing is 1250 ft. Total depth of the casing is 14,915 ft. The 9-5/8 inch diameter casing is set at 1980 ft. and there is no deep intermediate casing. A written plan of remediation is to be sent to Deputy Chief Dave Claus before any further steps to repair the well are taken.
0488423334 2014-08-04 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Drilling drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of casing O-Tex Cementing Service cemented with 270 sacks of class A cement at 15.6 lb. and 1.20 yield. During the job the conductor lifted 10 ft. in the air and the displacement rate had to be slowed to 1 barrel per minute. Had 5 barrels of cement returns.
0687163705 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

18 500 bbl. Production tanks 9 Producing wells 9 Sand separators

1 Horizontal heater-treater 1 Low pressure separator/ Vapor recovery system 2 Combustors 2 Compressors 4 300 Gallon Fuel Cells 1 Line Heater 3 NGL Production Vessels 3 NGL Compressors 3 NGL Heater Treaters

1004200304 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of a plunger lift system and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:

9- 500 barrel production tanks 9- Permitted well bores utilizing a plunger lift system 9- Gas production units equipped with internal heater units and sand traps 1- Low pressure separator 1- Combustor unit 1- Compressor 3- NGL tanks 3- NGL convertor units The site passed preliminary restoration in accordance with ORC 1509.072 A.

1708902440 2014-12-30 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by American Energy Utica consultant, John Wilson, that the Nomac 137 rig was currently drilling ahead through the 8.5 inch diameter section at a depth of 9,728 feet.
