0216359442 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of a plunger lift system and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:

9- 500 barrel production tanks 9- Permitted well bores utilizing a plunger lift system 9- Gas production units equipped with internal heater units and sand traps 1- Low pressure separator 1- Combustor unit 1- Compressor 3- NGL tanks 3- NGL convertor units The site passed preliminary restoration in accordance with ORC 1509.072 A.

0439416527 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

18 500 bbl. Production tanks 9 Producing wells 9 Sand separators

1 Horizontal heater-treater 1 Low pressure separator/ Vapor recovery system 2 Combustors 2 Compressors 4 300 Gallon Fuel Cells 1 Line Heater 3 NGL Production Vessels 3 NGL Compressors 3 NGL Heater Treaters

0813166621 2015-01-05 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by American Energy Utica (AEU) consultant, Russ White, that the 5.5 inch production string had been set the previous night. The Nomac 137 rig was in the process of rigging up on the current and last well on the pad (1H). Russ is anticipating that the surface job will take place the following day. While on site AEU Environmental Health and Safety representative, Tom Bauer, arrived on site to determine what could be done about the large pools of water present around the site from rain events. I field tested the water outside of the containment for the stockpile of dry chemicals and found the pH to be 5 and the chlorides to be 455ppm. I informed them that I could not authorize the release of the water at this time.
1325494954 2015-01-06 Casing String Witnessed No Violations On the date of inspection I was on site to witness the cementing of the surface casing. I was informed by American Energy Utica (AEU) consultant, Russ White, that the Nomac 137 rig had drilled a 17.5 inch diameter hole to a depth of 604 feet. All depths are measured from the Rotary Kelly Bushing at a height of 30 feet above ground level. Wyomoing Casing Service set 13.375 inch diameter, 54.50lbs/ft casing to a depth of 602 feet. The rig established circulation after setting casing before rigging up the cementers. O-Tex Pumping, LLC pumped . The job took much longer than expected due to freezing issues. The temperature was approximately 13 deg. Fahrenheit during the job with a sub zero wind chill.
1406989291 2014-08-02 Casing String Witnessed No Violations On the day of my inspection I found Precision Rathole Drilling installing 30 inch diameter casing at 100 feet in depth. O-Tex will be doing the cementing to surface. Established circulation and circulated 24 bbls of returns to surface.
1422040284 2015-01-22 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the production casing. This is the last cement job to be completed on the BSA Pad by the Nomac 137 rig at this time. I was informed by American Energy Utica consultant, Buzz Glover, that the Nomac 137 rig had drilled an 8.75 inch diameter hole to a depth of 15,122 feet. Wyoming Casing Service set 20.00lbs/ft, 5.5 inch diameter casing on bottom. O-Tex pumped a lead of 520sk of 50/50 POZ with Class H cement, 13.5ppg, 1.51 yield. O-Tex then pumped a tail of 2310sk of Class H cement, 15.3ppg, 1.27 yield. O-Tex displaced cement with 353bbl of water. Plug was bumped, floats held, and 58 of 70bbl of spacer returned to surface but no cement. The Nomac 137 will begin to rig down to move off location.
2014080155 2015-05-19 Preliminary Restoration No Violations On the date of inspection I am on site to conduct an inspection for preliminary restoration. The well passes for preliminary restoration per ORC 1509.072(A).
