0835300079 2014-08-07 Casing String Witnessed No Violations On the date of my inspection I accompanied ODNR Inspector Mark Moreland to witness the cementing of the conductor casing. Refer to Mark Morelands report for further details.
1014522434 2014-10-21 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by American Energy Utica consultant Brandon Gwin that the Nomac 137 rig had drilled an 8.75 inch diameter hole to a depth of 6750 feet and was in the process of loading the hole with mud.
1155559732 2015-05-19 Preliminary Restoration No Violations On the date of inspection I am on site to conduct an inspection for preliminary restoration. The well passes for preliminary restoration per ORC 1509.072(A).
1261768301 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of a plunger lift system and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:

9- 500 barrel production tanks 9- Permitted well bores utilizing a plunger lift system 9- Gas production units equipped with internal heater units and sand traps 1- Low pressure separator 1- Combustor unit 1- Compressor 3- NGL tanks 3- NGL convertor units The site passed preliminary restoration in accordance with ORC 1509.072 A.

1407427102 2014-08-07 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30: inch diameter casing string. Precision Drilling drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of casing at 136 lb. per ft. O-Tex Cementing Service cemented with 310 sacks of class A cement at 15.6 lb. and a yield of 1.20 had good cement returns. Could not enter GPS readings because the footage location has changed and Columbus has not updated the new numbers at this time. I did check with Columbus to make sure that they had submitted the new location request for the # 2-4-6-and 8 wells on this pad.
1413318820 2014-10-14 Casing String Witnessed No Violations On the date of inspection I was informed by consultant Russ White that the Nomac 137 rig had drilled a 17.5 inch diameter hole to a total depth of 636 feet. Costvs Energy Services set 13.375 inch 54.50lbs/ft casing on bottom. O-Tex Pumping, LLC pumped 810sk of Class H cement, 15.6ppg, 1.2 yield, 2% CaCl, 0.4% C-41P, 0.25lbs/sk of Celloflake. O-Tex displaced cement with 92bbl of fresh water and bumped plug. 22bbl of cement returned to surface and stayed at surface while I was on site.
1414533005 2014-10-28 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by American Energy Utica consultant Buzz Glover that the Nomac 137 Rig had reached a total depth of 14994 feet and had cemented the 5.5 inch production string earlier in the day. The rig is currently moving to the next well on the site.
1818320184 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

18 500 bbl. Production tanks 9 Producing wells 9 Sand separators

1 Horizontal heater-treater 1 Low pressure separator/ Vapor recovery system 2 Combustors 2 Compressors 4 300 Gallon Fuel Cells 1 Line Heater 3 NGL Production Vessels 3 NGL Compressors 3 NGL Heater Treaters
