0153721433 2014-08-07 Casing String Witnessed No Violations On the date of my inspection I accompanied ODNR Inspector Mark Moreland to witness the cementing of the conductor casing. Refer to Mark Morelands report for further details.
1213532426 2014-09-16 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by company man Russ White that the Nomac 137 rig was drilling an 8.75 inch diameter hole at a depth of 4,547 feet at the time of my inspection. The French drain system at the end of the pad opposite the entry road was still an open trench with water collecting in it. I checked the water with a Quantab and found the chloride level to be at 613ppm. I instructed Russ White not to discharge the drain system and that he would need to have a vac truck haul the water to an approved disposal location.
1410463559 2014-09-11 Casing String Witnessed No Violations On the date of my inspection I was informed by company man Russ White that the Nomac 137 rig had drilled a 17.5 inch hole to a depth of 604 feet. Wyoming Casing Crew set 13.375 inch 54.60lbs/ft casing to a depth of 603 feet and will be set on bottom after the cement job. O-Tex Pumping, LLC (O-Tex) pumped 810sks of Class H cement weighing 15.6ppg with a yield of 1.2. Additives include 2% CaCl, 0.4% C-41P, and 0.25lbs/sk of Celloflake. O-Tex pumped 87.3bbl of fresh water to displace cement. The Nomac 137 rig was unable to circulate the well after pumping approximately 300bbl of fresh water in the well. O-Tex pumped 45bbl of fresh water and gel spacer and 140bbl of cement slurry before circulation was established. 28bbl of cement returned to surface.
1450235544 2014-09-23 Drill / Deepen / Reopen No Violations On the date of my inspection I was informed by company man Russ White that the Nomac 137 was drilling an 8.75 inch hole at a depth of 12,381 feet at the time of my inspection. The Nomac 137 rig had top drive problems the day before my arrival but Russ White anticipated that the rig would reach Total Depth later that night and would be preparing to run casing and cement. During my inspection I checked the status of the French drain that had been excavated. W. Pol Excavating had removed approximately 100 feet of the drain where there had been contamination. They removed the stone that had surrounded the drain and had placed it in on site roll away containers. At the time of my inspection W. Pol Excavating had replaced the French drain with new pipe and set a new catch basin. They brought in new stone to cover the pipe. According to Russ White the samples that had been taken around the drain where the oil spill occurred had tested negative for Total Petroleum Hydrocarbons (TPH).
1476673711 2016-12-07 Preliminary Restoration No Violations On the day of my inspection I witness the lease road was in good repair and the gates to the location were open due to service companies working on the well bore. Upon my arrival I witnessed that the wells on location have been put into production. The permitted well bore is producing off of a plunger lift system and with my inspection I did not noticed any annular pressure issues. The production equipment on site consists of the following:

9- 500 barrel production tanks 9- Permitted well bores utilizing a plunger lift system 9- Gas production units equipped with internal heater units and sand traps 1- Low pressure separator 1- Combustor unit 1- Compressor 3- NGL tanks 3- NGL convertor units The site passed preliminary restoration in accordance with ORC 1509.072 A.

1784918720 2015-05-19 Preliminary Restoration No Violations On the date of inspection I am on site to conduct an inspection for preliminary restoration. The well passes for preliminary restoration per ORC 1509.072(A).
2077813500 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

2110262652 2014-08-07 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Precision Drilling drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of casing at 136 per ft. O-Tex Cementing Service cemented with 310 sacks of class A cement at 15.6 and a yield of 1.20 had 15 bbls of cement returns.
