0162577908 2014-05-15 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 9-5/8 inch diameter casing string. Patterson Drilling Rig # 261 drilled a 12-1/4 inch diameter hole to 1812ft and set 1802 ft of casing. While running the casing the rig broke down and I was told it would be down only 1 hour. After waiting for 3 hours I was told it could be just a few more minutes and then was informed it would still be several hours. I left at that time.
0581503089 2014-05-14 Drill / Deepen / Reopen No Violations I did not receive a call for the cement job. They called another Inspector by mistake. Patterson Drilling Rig # 261 drilled a 17.5 inch diameter hole to 409 ft and set 405 ft of 13-3/8 inch diameter casing. O-Tex Cementing Service cemented with 380 of class A cement at 15.6 lbs and a yield of 1.2 and had 15 bbls of cement returns. Set 5/13/2014
0902730214 2016-02-25 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067212730000 34067212740000 and 34067213220000 are producing off of natural reservoir pressure. Permitted well number 34067212750000 is producing off of a plunger lift system. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 2 compressors 2 NGL tanks which were not connected on the day of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
