1127903626 2018-12-18 Production Wells No Violations On the day of my inspection I found this pad is now owned by Ascent Resources. The pad is now called the Domino pad. Pad is seperated between a production equipment pad and a wellhead pad. Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five GPUs, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Surface annular pressure was 50psi and intermediate annular pressure was 300 psi.

Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
1656990846 2014-05-05 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of HAD Drilling. The drilling rig has moved off site and conductor casing appears to be set. I do not know how much casing that they set because nobody is on site.
1726988560 2014-06-09 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspectors Dylan Pendleton and Brandon Trigg, I witnessed that H&P Flex Rig #287 had drilled a 17.5 inch diameter hole to the depth of 1933feet and ran 1923 feet of 13.375 54.5#/ft. J-55 Casing. H&P had to wash down the last two joints of casing. Halliburton was on site and gained circulation with 10bbls of fresh water. Halliburton pumped 25bbls of gel; 10bbls of fresh water dropped the bottom plug and pumped 47 sacks of class A lead cement. Halliburton pumped 810 sacks of Class A tail cement and displaced cement with 285bbls of fresh water. Good returns of 65bbls of cement, plug was landed at 1100psi and pressured up to 1410psi for 5 minutes and the floats held. Cement stayed to surface.
1926360733 2014-06-09 Casing String Witnessed No Violations Onsite with Inspectors Doug Bennett and Brandon Trigg to witness cementing of the first intermediate string. H&P has set 1900 of 13.375 (61#/ft) as the first intermediate. Halliburton was onsite to cement the string with 50 sacks of Halcem system (15.6 ppg; 1.18 yield) as the lead cement and 810 sacks of BondCem System (15.8; 2.31 yield) as tail cement. 40 of cement was left in pipe as a shoe joint per Halliburton cement ticket. Good returns were viewed in cellar. See inspector Bennetts report this date.
