0489110260 2017-08-31 Preliminary Restoration No Violations On the day of my inspection I found the Cadiz D pad seperated between a production equipment pad and a wellhead pad. Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five line heaters, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.

1400198475 2014-05-15 Casing String Witnessed No Violations Arrive today to witness the running and the cementing of the casing string and found that HAD Drilling is broke down and will be ready at 4 this afternoon. Return at 4pm and found that they still do not have everything fixed and now the cement job will be 7 am Fri morning.

Established circulation but did not see cement returns to surface. Going to run bond log and then grout to surface.
1402606865 2014-06-12 Casing String Witnessed No Violations Onsite with Inspector Mark Moreland to witness the cementing of the intermediate casing. H & P had drilled a 17.5 open hole to a depth of 1926. They had 1931 of 13 3/8 casing set from the rig floor. Halliburton was onsite to cement this intermediate casing with a class H cement (15.8 ppg; 2.31 yield). Inspector calculations indicate 850 sacks of cement and 292 barrels to displace. Good return were viewed in the cellar. See his report for this date.
