0301211061 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains nine properly labeled producing wellheads utilizing a plunger lift system. Production equipment includes eight 500 barrel steel tanks in their own polymer lined steel containment, nine GPUs, nine sand seperators, three horizontal dehydrators, a compressor, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 150. Production records are current in RBDMS.
0385299059 2014-08-22 Casing String Witnessed No Violations On the date of inspection I was informed by the company man on site, Cecil Lovel, that the Patterson 256 drilled a 12.25 inch diameter hole was drilled to a depth of 7,121 feet. 9.625 inch casing weighing 40.0 lbs/ft was set on bottom. O-Tex Pumping, LLC pumped a lead cement of 640 sks low density cement at 12.0ppg, 2.15 yield, 5% Gypsum, 1% C-15, 0.6% C-20, 0.4% C-41P, 0.1% C-51. O-Tex then pumped a tail cement consisting of 210 sks Premium Class H with a weight of 15.3ppg, yield of 1.27, 5% KCl, 0.6% C-12, 0.4% C-41P, 0.1% C-51, 0.1% C-35. The total displacement was 535.6bbl of KCl water. The plug was not bumped. Held pressure for 5 minutes, floats held. Weatherford then opened the diversion tools at 3800 feet and began to circulate.
0499837988 2014-08-20 Drill / Deepen / Reopen No Violations I am on site today with Inspector Brandon Trigg and while waiting for a conductor cement job on this location we found chlorides in the 400 to 500 PPM range near the chemical containment area. The duck pond containment was broken down on 2 sides and rain had floated the chemicals in the open bags out to the secondary area of containment. One of the rig hands was pumping the rain water out of this area when we stopped him and had them pump the fluids into the holding tanks because of the chlorides. I met with Cecil the company man and had him make repairs to the chemical duck pond and to some to the main containment near the front of the rig where it has been torn.
0506209707 2014-08-21 Drill / Deepen / Reopen No Violations I am on site today with Supervisor Rocky King. We found that Patterson Drilling Rig # 256 is at 7000 ft. on the 12-1/4 inch diameter section of the well. They should be ready later tonight to run 9-5/8inch diameter casing and cement with a two stage DV Tool.

1391990567 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witness the well head is equipped with a production tree with valves in the open position. The production equipment included a sand trap and a GP unit. Also present were 3 stabilizer units, 2 line heaters, 2 compressor units, 3 NGL tanks, 2 combustors, 8- 500bbl condensate tanks and 4-500bbl brine tanks all within proper containment. This equipment is shared with the 7H, 5H, 3H, 1H, 2H, 4H, 6H and 8H. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1407967630 2014-08-13 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. Patterson Drilling Rig # 256 drilled a 17.5 inch diameter hole to 933 ft. and set 933 ft. of casing. O-Tex Cementing Service cemented with 990 sacks of class H cement at 15.6 lbs. and a yield of 1.20 and had 39 bbls of cement returns to surface.
1408626654 2014-08-12 Drill / Deepen / Reopen No Violations Met today with Stan Cherry per his request to make changes to the permit which was issued. They want to revise the casing program to be able to set the surface casing to 1000 ft and the intermediate casing to 7200 ft. which they have done on the last two wells.
1443715087 2015-10-01 Production Wells No Violations I received a call from Assistant Chief Scott Kell to advised me that a seismic event has taken place on the Hamilton Pad. Scott ask me to find out exactly what was taking place on that pad around 10 A.M. I arrived on the Hamilton Pad and found that there is a frack operation set up and ongoing. I met with Company Man Eric Boone who is in charge of the frack operation Mr. Boone advised me that they have just finished the frack on stage 20 of the # 10 well. I advised him of the event and he was unaware that a seismic event had occurred. He told me that Matt Warner is in charge of any information pertaining to seismicity and that Matt was in Oklahoma City. Eric said that he will contact Matt to advise him. He is to contact Assistant Chief Scott Kell to bring both groups information into synchronicity.
