0406367132 2014-07-10 Casing String Witnessed No Violations I am on site today to witness the 2nd stage of a two stage cement job. The first stage used 1340 sacks of class H cement at 12 lb and a yield of 2.15 for the lead cement. A tail cement of class H at 15.3 lb and a yield of 1.27 with a total of 210 sacks. After the cement was displaced a ball was dropped to open the DV Tool which is at 3610 ft. All of the cement in the annular space was circulated to surface. Circulation continued for 6 hours and then the 2nd stage filled the annular space from 3610 ft to surface. O-Tex Cementing Service used class H cement at 12 lb and a yield of 2.15 a total of 740 sacks was used. 74 bbls of cement circulated to surface. The plug was bumped at 500 PSI over displacement pressure and held when released.

1036742379 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains nine properly labeled producing wellheads utilizing a plunger lift system. Production equipment includes eight 500 barrel steel tanks in their own polymer lined steel containment, nine GPUs, nine sand seperators, three horizontal dehydrators, a compressor, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.
1160316843 2016-03-01 Preliminary Restoration No Violations This well is in production and meets the requirements for the preliminary restoration. The well is one of 9 wells in operation on this pad and has proper identification. To see a list of equipment used to operate this well see permit # 1366
1256393361 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witness the well head is equipped with a production tree with valves in the open position. The production equipment included a sand trap and a GP unit. Also present were 3 stabilizer units, 2 line heaters, 2 compressor units, 3 NGL tanks, 2 combustors, 8- 500bbl condensate tanks and 4-500bbl brine tanks all within proper containment. This equipment is shared with the 7H, 5H, 3H, 1H, 2H, 4H, 8H and 10H. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1397071165 2014-04-09 Casing String Witnessed No Violations I am on site today to witness the running and cementing of the 20 inch diameter casing string. Express Drilling drilled a 26 inch diameter hole to 105 ft and set 105 ft of casing. Express Cementing Service cemented with 200 sacks of class A cement at 15.6 lbs and 1.20 yield . Circulate 9 bbls of cement to surface.
1404958373 2014-07-07 Drill / Deepen / Reopen No Violations I am on site tonight to witness the rnnning and the cementing of the 9-5/8inch diameter casing string. Patterson Drilling has casing at 7100 ft and is trying to wash casing to bottom. They could not get casing to total depth and will cement it where it is. They will circulate bottoms up 1.5 times and then rig up to cement. They are to call me at that time.
1405002816 2014-07-09 Casing String Witnessed No Violations I arrived today to find that Patterson Drilling Rig # 256 was not able to wash casing down. They have the 13-3/8 inch diameter casing at 7095 ft. O-Tex Cementing service cemented with 1340 sacks of lead cement at 12.0 lbs and a yield of 2.15 tail cement of 210 aacks of class H cement at 15.3 lbs and a yield of 1.27. Next the D.V.Tool at 3610 ft was pumped open and all of the cement was circulated out of the annulus to surface. They will continue to circulate for 6 more hours and then will cement again to surface.
1406199699 2014-07-23 Casing String Witnessed No Violations On the date of inspection I checked in with American Energy Utica, LLC Company Man Burl Miller. The Patterson 256 rig was at a Total Depth of 14,880 feet. 5.5 inch casing was seated on bottom within a 8.75 inch open hole up to 7,102 feet. I was present on site to witness the beginning stages of production casing cementing.
