0957365698 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067213270000 and 34067213260000 are producing off of natural reservoir pressure. Permitted wells 3406712970000, 34067212900000 and 34067213170000 are utilizing a plunger lift system production. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

9 400 bbl. Production tanks 5 Producing wells 5 Sand separators

1 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 1 compressor 2 NGL tanks.

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1399887838 2014-05-12 Casing String Witnessed No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #71 drilled a 12.25 inch diameter hole to the depth of 1975 feet. Wyoming Casing Crew ran and set 1958 feet of 9.625 inch 40#/ft J-55 casing. Schlumberger was on site and gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 690 sacks of Class A 15.6ppg, 1.23cuft/sack cement. Had good returns of 21bbls, plug was landed at 774psi, pressured to 1410psi for 5 minutes and the floats held.
1970300746 2014-05-10 Casing String Witnessed No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #71 drilled a 17.5 inch diameter hole to the depth of 655 feet. Wyoming Casing Crew ran and set 649 feet of 13.375 inch 54.5#/ft J-55 casing. Schlumberger was on site and gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 576 sacks of Class A 15.6ppg, 1.22cuft/sack cement. Had good returns of 28.5bbls, plug was landed at 270psi and pressured to 770psi for 5 minutes and the floats held.
