0814770251 2014-08-06 Drill / Deepen / Reopen No Violations On the date of inspection I accompanied ODNR Engineer Jason Simmerman to check the status of the construction that has taken place on site. The Company Man on site was Ryan Bartholomew. I was informed that the Nomac 245 rig had drilled to a total depth of 14,566 feet. At the time of my inspection the rig was preparing to start running 5.5 inch production casing. Refer to Jason Simmermans report for further details regarding construction.
1060545183 2016-12-15 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road in good repair and the gates to access the location were locked. With my arrival on location I witnesses I witnessed all wells on site are now in producing order. The permitted well bore is utilizing natural reservoir pressure for means of production. The production equipment that is on site consists of the following:

2- 500 barrel production tanks 7- Permitted well bores utilizing natural reservoir pressure 7- Gas production units equipped with internal heaters and sand separators 1- Low pressure separator system

The site passed preliminary restoration in accordance with ORC 1509.072 A.
1394148712 2014-03-03 Casing String Witnessed No Violations I am on site to witness the running and the cementing of the 24 inch diameter casing string. Butchs Rat Hole Service drilled a 30 inch diameter hole to 105 ft and set 105 ft of casing. Halliburton Energy Service c/w 290 sks of class A cement @ 15.5 lb and 1.22 yield and circulated cement to surface.

2146100805 2016-11-22 Production Wells No Violations On the day of my inspection I discovered Eclipse Energy in the finishing processes of putting the wells into production. While on site I spoke to a contractor worker who informed me all wells are tied into the sales line and are in producing order. I will be back on site at a later date to perform a visual inspection on the well bores and there production equipment. I was unable to capture this information due to the amount of people on site working on the well bore.
