0232968832 2014-05-05 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Patterson Drilling Rig # 256 I found that they are 7,000ft making a trip to pick up directional equipment. I met with Matt to talk about the fact that there was diesel fuel found where the sub base would have been on the last well. I went over some of the information as to how and where the spill could have entered the ground. We talked about point of contact for any of the contamination and how it could be eliminated on future drill sites.
0300716651 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witness the well head is equipped with a production tree with valves in the open position. The production equipment included a sand trap and a GP unit. Also present were 3 stabilizer units, 2 line heaters, 2 compressor units, 3 NGL tanks, 2 combustors, 8- 500bbl condensate tanks and 4-500bbl brine tanks all within proper containment. This equipment is shared with the 7H, 5H, 3H, 1H, 4H, 6H, 8H and 10H. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1077885713 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains nine properly labeled producing wellheads utilizing a plunger lift system. Production equipment includes eight 500 barrel steel tanks in their own polymer lined steel containment, nine GPUs, nine sand seperators, three horizontal dehydrators, a compressor, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 100. Production records are current in RBDMS.
1398615726 2014-04-27 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. Patterson Drilling Rig # 256 drilled a 17.5 inch diameter hole to 595 ft and set 588 ft of casing. O-Tex Cementing Service cemented with 553 sacks of class H cement @ 15.6 lb and 1.21 yield. Circulated 28 barrels of good cement to surface.
1398617352 2014-04-10 Casing String Witnessed No Violations I an on site today to witness the running and the cementing of the 20 inch diameter casing string. Express Drilling has a rig over the hole which broke down and has moved in a second rig. Before casing could be set it broke down. They did drill a 26 inch diameter hole to 105 ft and set 105 ft os 20 diameter casisng. Express Cemented the casing with 200 sacks of class A cement @ 15.6 lb and 1.18 yield. Had cement returns.
