0842500868 2014-06-12 Casing String Witnessed No Violations Onsite to witness an intermediate string cement job. Patterson rig 261 has drilled a 12 1/4 open hole to a depth of 1810. The rig set 1800 of 9 5/8 (40#/ft) casing. Otex cementing is onsite with 690 sacks of class H cement with a weight of 15.8 ppg and yield of 1.21. Cement returns were viewed in the cellar.
1402502523 2014-06-11 Casing String Witnessed No Violations Accompanied By Inspectors Mark Moreland and Brandon Trigg. Patterson 261 drilled a 17.5 open hole and set 410 of 13 3/8 (54.5#/ft). Otex pumping LLC is onsite to perform cement job. 15 bbl of cement returned to the surface.
1415741631 2018-11-06 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad containing 4 producing wells. This well is equipped with a plunger lift system operated by a motor valve. Surface annular pressure is 0 and intermediate annular is 445. Production equipment includes 4 GPUs, 4 sand seperators, 1 compressor, a combustor, vapor recovery tower, and 8 400 barrel steel tanks in their own containment. Five tanks are condensate and three are produced water. Site meets requirements of OAC 1509.072A for preliminary restoration.
1946297253 2014-03-19 Drill / Deepen / Reopen No Violations Check on equipment which is on site and found that they are getting site ready for a drilling rig to move in, in less than a week. Conductor was set on two of them but they need to set two more.
2015082131 2016-02-25 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067212730000 34067212740000 and 34067213220000 are producing off of natural reservoir pressure. Permitted well number 34067212750000 is producing off of a plunger lift system. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 2 compressors 2 NGL tanks which were not connected on the day of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
