0048206543 2014-05-29 Casing String Witnessed No Violations On arriving on location Patterson rig # 261 was running 9 5/8 inch casing to the depth of 1804 feet. O-Tex will be cementing to surface. Established circulation, circulating 26 bbls of good cement returns to surface.
0163330962 2018-11-06 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad containing 4 producing wells. This well is equipped with a plunger lift system operated by a motor valve. Surface annular pressure is 0 and intermediate annular is 500. Production equipment includes 4 GPUs, 4 sand seperators, 1 compressor, a combustor, vapor recovery tower, and 8 400 barrel steel tanks in their own containment. Five tanks are condensate and three are produced water. Site meets requirements of OAC 1509.072A for preliminary restoration.
0227498211 2016-02-25 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067212730000 34067212740000 and 34067213220000 are producing off of natural reservoir pressure. Permitted well number 34067212750000 is producing off of a plunger lift system. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 2 compressors 2 NGL tanks which were not connected on the day of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1401255241 2014-05-28 Casing String Witnessed No Violations I am on site to witness the running and cementing of the 13-3/8 inch diameter casing string. Patterson Drilling Rig # 261 drilled a 17.5 inch diameter hole to 405 ft and set 405 ft of casing. O-Tex Cementing Service cemented with 390 sacks of class A cement at 15.6 lb and a yield of 1.2 and had 15 bbls of good cement returns.
1401259978 2014-05-28 Casing String Witnessed No Violations Accompanied Inspector Mark Moreland to witness a 13 3/8 surface casing job. See his inspection report for this date. Calculated cement volumes, depth of hole (405) times annular space (.6946) x excess cement (50%) divided by 1.18. Also calculated displacement volumes using capacity of casing and linear ft per barrels.
