0241822140 2014-05-07 Drill / Deepen / Reopen No Violations I am on site today to check on the cleanup status. I found Hull and Associates on site and met with them to go over the details of their investigation. The east pit is almost dry and the west pit is dry. They are setting up to capture the contamination during and after the rain.
0692157982 2014-04-17 Drill / Deepen / Reopen Violations Noted My inspection today is to find out more about what has caused the slip on the left side of the entrance to the drilling pad. I have found that the 450 PPM chlorides reported to me was actually 4500 PPM Mixed with the chlorides is some type of oil either mineral oil or oil base drilling mud leaking from the slip. I am on site today with Blake Arthur from ODNR Columbus office. Blake was called to help evaluate the cause of the slip and to advise me on how the fluids may have dispersed through the drilling pad. Hull and Associates will also help to determine how the fluids are impacting the site and the surrounding area. I will wait until further investigation has been done to talk with the Operator as to how to stabilize the site and begin the clean up of the chlorides and other substance. The slip at the east and the west shall be stabilized. All contaminated fluids shall be captured and contained on site. Contaminated soil shall be hauled to a landfill and fluids to a disposal well. Further details shall be included at a future date.
0911256422 2016-02-08 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap and a gas production unit. Production equipment that was shared with permitted wells 34067212700000 and 34067214720000 included a vapor recovery system, a gas compressor, a gas combustor, a line heater, 2 NGL tanks and 10-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1390315045 2014-05-05 Drill / Deepen / Reopen No Violations I am on site today and found that Hull and Associates Ryan Hegburg was on site working on Geo Drilling data along with the delineation process for finding where and how the spill occurred. The chlorides at the east and west ends of the location have dropped in flow rate and chloride range. The amount of flow from the east side is down to less than few barrels a day. No source has been found for the chlorides at this time.
1396461047 2014-04-02 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 10-/3/4 inch diameter casing string. Patterson Drilling Rig # 256 drilled a 14-3/4 inch hole to 2110 ft and set 2100 ft of casing. O-Tex Cementing Service cemented with 290 sacks of class A cement @ 15.2 lbs and a yield of 1.31 of Lead cement and 1010 sacks of class H cement @ 15.8 lb and a yield of 1.21 tail cement and circulated good cement to surface. Randy Akins was on site to witness as part of his training.
1398037937 2014-04-14 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Patterson Drilling Rig # 256 and found that they are at 14,500 ft drilling a 8.5 inch diameter hole. They should be running and cementing production casing Wed. While on site today I was advised that a slip had occurred on the left side of the drilling pad near the base. It is on the same side as the entrance. I did check on the site and found that Hull and Associates are on site to evaluate the slip and to make corrections. During my inspection of the slip I was advised that there were chlorides in the 450 PPM range. I did an inspection of the containment and did not find any leaks. I talked with Chris Burt and Stan Cherry of American Energy Partners and advised them to use dye packs to test the containment for leaks. They planned to test the fuel and the containment liner.
1398042445 2014-04-16 Drill / Deepen / Reopen No Violations I am on site today to check on the slip on the left side of the drilling pad and the containment for fluids which are coming from the slip. I found that the containment is in place and appears to be collecting the fluid which is being returned to one of the tanks on site. I found that the level of the chlorides has remained at the same range which is 4500 PPM. Since they have not declined a larger leak may have occurred. I will contact Blake Arthur to ask for help in making a determination as to where the fluid may migrate and also determine a source. I advised AEU that the Compliance Notice would be issued after my meeting with Blake Arthur.

1856397631 2016-01-04 Production Wells No Violations I am on site today conducting a follow up inspection on NOV No. 692157982 that occurred on 04/17/2014. The NOV was issued due to chloride contamination that occurred on the slip of the East and West side of the location. With my inspection today I walked the whole perimeter of the well pad where I found no signs of chloride contamination and found the area surrounding the pad had well established vegetation. It should be noted that the NOV is now resolved.

2131198569 2014-04-27 Drill / Deepen / Reopen No Violations I met on site today with Ryan Hegburg and went over what is going on with the clean up of the site. He said that the rate of flow has dropped to a trickle and that the chlorides have dropped as well. The east side is down below 200 and the west side is at 1200 PPM. The plastic matting boards have been removed and the liner has around 22 holes. It is to be rolled off Monday and we should be able establish where the leak began.
