0126178709 2013-11-27 Drill / Deepen / Reopen No Violations Auger Drilling rig is on site and they are broke down at this time. Return after they were able to get casing in the well at 110 ft and now wait on cement trucks. Trucks did not arrive as scheduled and after a few hours I did not stay because I did not know how long it would be before the trucks arrived.
0146333269 2014-01-13 Drill / Deepen / Reopen No Violations I received a call Sunday evening from Chris Anderson who advised me that an oil sheen was witnessed in the left corner sump in the 12 inch casing. He said that they did not know what or how and that it was only a trace floating on the top of the water in the sump. I contacted Doug Bennett and relayed that information and I am on site today to check how much is floating on the surface. I found that it was not solid and only a trace was observe red. I contacted Doug and advised him what I had found . He said that he is going to collect a sample to check for chlorides and see if a sample for the lab is needed.
0190813857 2013-12-24 Drill / Deepen / Reopen No Violations On the day of my inspection Sidewinder Drilling Rig #121 was mist drilling with an 8.75 inch diameter bit at 6133 feet continuing to their lateral KOP of 7300 feet. Site inspection revealed that rain water has been removed and containments were in good shape with no violations.
0251440307 2013-12-02 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that conductor pipe has been set and secondary containment was in place with berms being built upon arrival. Site inspection revealed that the pad liner had been torn during snow removal and request was made to containment crew to repair liner once berms were built and before rig was moved to site.
0415459161 2014-01-13 Drill / Deepen / Reopen No Violations On the day of my inspection Sidewinder Drilling rig #121 has begun to rig down with moving to take place to the Milliken Pad on Wednesday. Wyoming casing crew set 17701 feet of 5.5 inch production casing and CalFrac cemented the casing with 112 sacks of scavenger 50/50 cement, 1256 sacks of 50/50 cement and 2268 sacks of Class H cement. During displacement lost circulation at 285bbls of 374bbls. I discussed the job with Company Man Dean Cook and Drilling Engineer Carter Layfield and expressed concerns for top of cement, Carter stated that a cement bond log would be run before completing the well. I took a water sample out of the northeast sump on request from ALL Consulting due to oil sheen on the top. Upon testing sample chlorides were normal and PH was 6. Also took an absorbent pad and retrieved no noticeable oil on the pad. Upon further site inspection revealed no violations.

Met and discussed upcoming casing program with Gulfport Company Man Dean Cook. Rig should spud tomorrow with surface casing to follow. Site inspection revealed no violations.
0531715855 2013-12-13 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of the Nabors Drilling Rig # X07 and found them rigging down for repairs to the sub soil below the drilling rig. Conference with Blake Arthur and Ron Trivisonno about the repairs which would need submitted to the Division. Chevron has 3 Soil Engineers which will review the soil information and submit a remedial repair plan to Ron Trivisonno to review and approve before drilling continues.

1038373750 2015-07-28 Production Wells No Violations I am on site today to review a condensate spill which occurred on July 6 -2015. The inspection date was on July 7- 2015. The excavated material which was stored in 3 roll off containers from that date has been removed to a landfill. The area has been back filled and graded. The clean up of this site is complete and the wells are back in operation.
1237703165 2017-09-06 Production Wells No Violations On the day of my inspection I found this was the only well on the pad. Well is not showing any pressure on surface or intermediate casing. Production equipment includes five 400 steel condensate tanks, one 500 barrel steel condensate tank, three 400 barrel steel brine tanks, one 50 barrel steel brine tank, gas production unit, vertical heater treater, and vapor recovery system, and combustor. All tanks are enclosed in an steel dike. Production is current in RBDMS.
1359994553 2014-01-06 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Sidewinder Drilling Rig #121 was at 16127 feet and had lost circulation. The second LCM pill had just been released and being displaced trying to locate and heal the thieving formation. When circulation is regained will continue drilling to an estimated TD of 17700 feet with 5.5 inch production casing to follow.
1385386434 2013-11-25 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status. Redstone Drilling is drilling the conductor hole and should be ready today to cement the conductor today.
1385477027 2013-11-25 Drill / Deepen / Reopen No Violations I returned to check on the drilling status of Redstone Drilling. During my inspection in the morning I discovered that they had encountered a void at 30 to 34 ft which was not known as to what it was. They thought that it may just be a bolder or fill space in an old mine site. They used 7 yards of grout cement to try and fill and stabilize the void. It did not fill it up and another 7 yards of grout cement was ordered. I was on site during the grout job and it did not fill the void in. I contacted our Columbus office and found that the location is 1000 ft from an old coal mine. I think that the void is an auger hole not a mine shaft. I advised Nick Beresman that he could place a 30inch diameter joint of casing in the well and cement it up the back side of the casing. Then they could drill and set the conductor as normal and cement it to surface. Nick said that he would try one more load of cement grout to fill it in. Nick called me later and informed me that the grout had filled in the void and he would core drill the cement in the void and contact me when he was ready to set conductor.
1385488424 2013-11-26 Drill / Deepen / Reopen No Violations I arrived today to check on the drilling status of the conductor hole. I found that Redstone Drilling is at 38 ft drilling what they said is new material and what looked like cement to me. They could set casing later today or tonight. The weather is going to be a problem as the snow continues to fall. Nick Beresman is to call when they get ready to cement casing.
1387029810 2013-12-14 Casing String Witnessed No Violations

-55 casing. Stingray Cementing LLC was on site and gained circulation 10bbls of fresh water, 25bbls of gel, 10bbls of fresh water and cement casing with 554 sacks of Class A, 15.6lbs/gal., 1.19cu.ft/sk., 5.2gal/sk. Had good returns of 3bbls and plug landed and pressured up to 697psi and held for 5 minutes.

1426506422 2015-03-03 Production Wells No Violations My inspection of this facility revealed no leaks or spills. The production tanks are inside a lined dike that appears to be liquid tight. The well has a tree in place and production continues through line heaters and separators with a flare stack. There are also solar panels in place to operate the well using telemetry.
1436360647 2015-07-06 Production Wells No Violations I received a call from Brad Burwell around 3:15 PM on Monday 7/06/2015 to inform me that a 20 gallon condensate spill had occurred. The seal on the sight glass tube developed a leak and allowed condensate to mist under pressure around the Vertical Heater Treater. It placed a mist around the Heater Treater in a 20 ft. circle with more going toward the north due to wind. Brad told me that he had equipment mobilized to respond to the spill and that if they could they would begin before dark. He advised that the spill was mostly mist and in the middle of the pad with no chance of it to get off site. They also had vacuum trucks rolling toward the site and would be able to vacuum any liquids before they could move off site. I contacted Supervisor Brent Bear to advise.
1443184761 2015-09-24 Production Wells No Violations At 1:30 PM Brad Burwell of Gulfport Energy called me to advise me that a fire had occurred on the Michaels Pad and that the fire was out. The fire occurred on the emergency flare stack. He said that the fire dept. had not been called because they were able to close the valves on the system which stopped the fire. I notified Region Supervisor Brent Bear and Scott King about the fire. Next I responded to the fire site and found that the fire was out and everything had been contained on site. The fire was caused after the compressor had been serviced. The hydraulic oil had been changed and drained into the low pressure vapor recovery system. That system has a dump line which was where the oil was intended to go. There is a check valve on the low pressure vapor recovery separator that failed. It allowed the oil to be sent to the vapor recovery flare stack instead of the dump side. The hydraulic oil mixed with low levels of condensate and was ignited by the flare. It cascaded down the flare stack to the ground and continued to burn until the valves on the system were closed. The fire then burned itself out. There is about 1/2 gallons of hydraulic oil on the ground which needs cleaned up and removed. The fire did not do any damage to the flare and after the lines have been cleared of fluid can be placed back in operation. There has been another check valve added to this system as a back up to keep it from happening again. Also the valve which failed should be replaced. The field people on site for Gulfport were James Noyes and Andy Baker along with Brad Burwell.
1478036294 2015-09-25 Production Wells No Violations I returned to this site to follow up on the fire and clean up which occurred on 9/24/2015 I met on site with Andy Baker Gulfports pumper for this well. I found that the hydraulic oil has been cleaned up and removed to a roll off container on site. The check valve which failed has been replaced and the line from the low pressure separator to the flare has been vacuumed clear of fluid. The wells are scheduled to be back on line by noon today. All of the clean up and repairs have been completed.

The rig has not arrived and there are only a few people on site. I spoke with those people on site and found out the rig has had some moving problems with the permit and will move after they are corrected.

2048172036 2015-07-07 Production Wells No Violations I arrived on site today to find that a clean up crew has removed 3 roll off containers full of contaminated rock and soil. The roll off containers are 15 cubic yards each for a total of 45 yards of material. The containers are covered and the condensate contaminated material will remain on site until the samples of the contaminate are confirmed. The area around the Vertical Heater Treater has be excavated and clean fill along with new limestone put back and graded. The repairs to the sight tube and valves have been made and the well will be back on line this evening. Hal Miller is on site for Gulfport and has used a new PID meter to screen the soils and excavated material. The clean up is complete at this time and Hal and myself used the meter in and around a large area to confirm the condensate has been removed.
