0348732830 2013-10-09 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Precision Drilling drilled a 26 inch diameter hole to 30 ft and a 24 inch diameter hole to 120 ft. They set 120ft of casing and Allied Cementing Service c/w 250 sks of Class A cement @ 15.6 lbs and 1.18 yield. They had 10 bbls of good returns.
1390799356 2014-01-27 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #71 drilled a 17.5 inch diameter hole to the depth of 735 feet. Wyoming Casing Crew ran and set 730 feet of 13.375 inch 54.5#/ft J-55 casing. Schlumberger was on site and gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 660 sacks of Class A 15.6ppg, 1.20cuft/sack cement. Had good returns of 37bbls, plug was landed and pressured to 865psi for 5 minutes and the floats held.

1803368639 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. Permitted well numbers 34067212490000, 34067211940000 and 34012510000 are utilizing a plunger lift system for means of production. While permitted well 34067212500000 is producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Gas compressor unit. 2 Horizontal heater-treater. 2 NGL tanks that were disconnected. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1961943044 2014-01-29 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #71 drilled a 12.25 inch diameter hole to the depth of 1930 feet. Wyoming Casing Crew ran and set 1919 feet of 9.625 inch 40#/ft J-55 casing. Schlumberger was on site and gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 705 sacks of Class A 15.85ppg, 1.1.18cuft/sack cement. Had good returns of 30bbls, plug was landed, pressured to 1259psi for 5 minutes and the floats held. Site inspection revealed no violations.
2078471400 2014-02-11 Drill / Deepen / Reopen No Violations On the day of my inspection Nomac Drilling Rig #71 drilled an 8.5 inch diameter hole to the vertical depth of 17088 feet. Wyoming Casing Crew ran and set 17088 feet of 5.5 inch 20#/ft. P-110 casing. Schlumberger was on site and gained circulation and pumped 60bbls of mud push and cemented casing with 900 sacks of Class A 13.5ppg, 1.67cuft/sack lead cement and 2160 sacks of Class A 15.0ppg, 1.30cuft/sack tail cement. I witnessed that the cement was displaced with 379 of fresh water; plug was landed, pressured to 2409psi for 5 minutes and the floats held. Casing slips and night cap are being installed and rigging down has begun.
