0288936055 2014-01-16 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A
0521261078 2014-01-02 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #71 drilled a 12.25 inch diameter hole to the depth of 1945 feet. Wyoming Casing Crew ran and set 1945 feet of 9.625 inch 40#/ft J-55 casing. Schlumberger was on site and gained circulation with 25bbls of gel followed by 10bbls of fresh water and cemented casing with 730 sacks of Class A 15.85ppg, 1.19cuft/sack cement. Had good returns of 24bbls and plug was landed and pressured up to 1467psi held for 5 minutes and plug held.
1389195066 2014-01-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #71 was at a depth of 13522 feet with an estimated td of 17073 feet. I notified Company Man Craig Coen that I would be the first call inspector checking for the rig. Site inspection revealed no violations.
1563106552 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. Permitted well numbers 34067212490000, 34067211940000 and 34012510000 are utilizing a plunger lift system for means of production. While permitted well 34067212500000 is producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Gas compressor unit. 2 Horizontal heater-treater. 2 NGL tanks that were disconnected. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

2007271947 2013-10-09 Casing String Witnessed No Violations Precision Drilling drilled a 26 inch diameter hole to 30 ft and a 24inch diameter hole to 110 ft and set 110 ft of 20 inch diameter casing. Allied Cementing service c/w 250 sks of class A cement and had 25 bbls of good returns.
