0256373620 2013-10-15 Drill / Deepen / Reopen No Violations Arrived on location for a cement job. Crew set 1,570 of 9 5/8 casing with 487 sks with good cement returns.
0587503958 2017-11-06 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain five properly labeled plunger lift wells. Production equipment includes five gas production units, five sand seperators, and two 400 barrel steel tanks inside steel containment. Well tender Cramblett said that oil is piped to a nearby facility. Site meets requirements of ORC 1509.072A for preliminary restoration. See permit 34-067-2-1247-01-00.
1381405455 2013-10-09 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Northest Drilling Rig # 12 and found that they are at 102 ft and are broke down. This is small rig and will only drill the surface and intermediate section.
1382443159 2013-10-11 Drill / Deepen / Reopen No Violations Arrived on location for surface job, company set 497of 13 3/4 casing with 397sks of cement good returns.
1416891967 2016-07-26 Preliminary Restoration No Violations I arrived on location where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of a plunger lift system. RBDMS indicated that the well is in producing order.
The production equipment on the pad consists of the following:

2 400 bbl. Production tanks 5 Producing wells 5 Sand separators

The site passed preliminary restoration in accordance with ORC 1509.072 A.

1793313802 2016-11-10 Production Wells No Violations On the day of my inspection I witnessed that the permitted well bore has an incorrect API number listed on the ID Tag located both at the well head and at the end of the lease road. After witnessing the well is missed identified I place a call to Tim Mchonehey (An Eclipse Manger) and informed him of my findings. Tim state that he was unaware of the improper identification and he would make sure that the right identification plates where installed on all well bores. While on site I performed routine inspections were I did not find any violations at the time of my inspection.
2071928608 2017-11-06 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain five properly labeled plunger lift wells. Production equipment includes five gas production units, five sand seperators, and two 400 barrel steel tanks inside steel containment. Well tender Cramblett said that oil is piped to a nearby facility. Intermediate casing annular pressure is 200 and the surface casing annular pressure is 0. Production records up to date in RBDMS. Site meets requirements of ORC 1509.072A for preliminary restoration
