0499942247 2015-02-10 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They have reached the landing point of 8,586 and are currently drilling the 8.5 lateral at a total measured depth of 9,421. They mudded up at 7,245 and expected total measured depth is 17,824. All drilling equipment is on containment and the location is in good condition. No violations.
0846893003 2015-02-13 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. The rig is currently drilling the 8.5 lateral at a total measured depth of 16,978 and should reach total depth tomorrow (2/14/15). All equipment is on containment and the location is in good condition and icy. No violations.
1533892403 2015-02-01 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac Rig #73 spudded the surface hole on 02/01/15, drilled a 17.5 hole to a depth of 531, and Wyoming Casing ran surface casing to a depth of 521. O-Tex rigged up, tested the lines, and pumped 30 barrels of gel and 10 barrels of water ahead of the slurry. They then mixed and pumped 102 barrels of cement slurry, dropped the plug, and displaced the plug to the collar with 74 barrels of water. The plug landed at 250psi and the pressure was increased to 750psi and held for 5 minutes. The pressure was released, and the floats held with 0.5 barrels flowing back to the truck. 14 barrels of cement circulated to surface and weighed 15.5lbs/gal on the pressurized scales. See casing ticket for more details on this job. No violations.
1794594137 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 5 compressors

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1821323255 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap, a gas production unit and a line heater. Production equipment that was shared with permitted wells 34067212280000, 34067212290000, 34067212300000, 34067212310000, 34067212320000, 34067214310000, and 34067214320000 included a vapor recovery system, 5 gas compressors, a gas combustor and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1861336807 2014-12-20 Casing String Witnessed No Violations The Express rig drilled 120 of 26 hole this date and ran 120 of 20 casing. Express cementing cwemented it with 230 sacks of Class A cement. See the casing ticket for the details.
