0276947295 2015-04-03 Casing String Witnessed No Violations I was on the Klodt # 8 H to witness the placement and cementing of the 13.375 surface casing. Nomac Drilling Rig # 73 drilled a 17.5 borehole from 120 KB to 570 KB. Wyoming Casing Service ran 12 joints of 13.375, 54.5 lb/ft, BT&C, J-55 casing, five (5) bow spring centralizers, a float shoe, a float collar, a casing hanger and a landing joint. Total tally ran was 529 set at 557 KB. Baker Hughes conditioned the borehole with 10 barrels of water followed by 25 barrels of gelled spacer. Baker Hughes mixed a cement slurry consisting of 489 sacks ( 104 barrels ) of Class A blended with 0.05% SF, 2% CaCl and 0.25 lb/ft celloflake. Cement slurry weight (verified by scale) was 15.6 ppg. The cement slurry was displaced using a top plug and 79 barrels of water. The plug was landed at 210 psi and increased to 814 psi for five (5) minutes without any pressure decline. Baker Hughes bled off the pressure and one (1) barrel flowed back and stopped. All floats held. Cement slurry returns weighed 15.2ppg ( verified by scale ) and were approximately 29 barrels . No violations were identified.
0440780791 2015-04-04 Casing String Witnessed No Violations I was on the Klodt # 8 H location to observe the placement and cementing of the 9.625 intermediate casing. Nomac Drilling Rig # 73 drilled a 12.25 borehole from 530KB to 2400 KB. Wyoming Casing Service ran 54 joints of 9.625, 40 lb/ft, LT&C, J-55 casing, 19 bow spring centralizers, a float shoe, a float collar, a casing hanger and landing joint. Total tally ran was 2,387 set at 2414 KB. Baker Hughes conditioned the borehole by pumping 10 barrels of water and a 25 barrel gelled spacer. Baker Hughes mixed a cement slurry consisting of 879 sacks ( 184 barrels ) of Class A cement blended with 0.05% SF, 0.5% CaCl, 0.3% A10. The cement slurry was displaced back to the cellar utilizing a top plug and 176 barrels of water. The plug was landed at 906 psi and increased to 1516 psi. After being held for five (5) minutes without any pressure decline Baker Hughes bled off the pressure. All floats held. Thirty five ( 35 ) barrels of cement slurry was returned to the cellar. No violations during this inspection.
1010717941 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 5 compressors

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1204808350 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap, a gas production unit and a line heater. Production equipment that was shared with permitted wells 34067212290000, 34067212300000, 34067212310000, 34067212320000, 34067212330000, 34067214310000, and 34067214320000 included a vapor recovery system, 5 gas compressors, a gas combustor and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
1419857016 2014-12-20 Casing String Witnessed No Violations Express drilled 120 of 26 hole and ran 120 of 20 casing. Express cemented it with 230 sacks of Class A cement. See the casing ticket for the details.

Baker - Hughes cement with 488 sacks of Class H cement. Approximately 33 barrels of cement slurry ( 15.6 ppg ) were returned to the cellar. A walk around inspection revealed no violations during this inspection.
