0136929820 2016-02-17 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 4 Producing wells 4 Sand separators

2 Horizontal heater-treaters. 3 Compressors 2 NGL tanks There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

0666543412 2014-09-10 Casing String Witnessed No Violations This well was spud on 9/8/14. Universal cemented 120 of 20 94# casing with 445 sacks of Class A cement. 29 bbls of good cement was returned to surface. See the casing ticket for the details.
1420987545 2015-01-11 Casing String Witnessed No Violations Upon arrival on location I found the rig crew had installed 575 feet of 13 3/8 inch casing. Schlumberger will be cementing. Established circulation and circulated 27 bbls of good cement returns to surface.
1421158566 2015-01-13 Casing String Witnessed No Violations On arrival on location I found the crew had set 2002 feet of 9 5/8 inch casing. Schlumberger will cement. Established circulation and circulated 28 bbls of good cement to surface.
