0722672301 2016-09-07 Production Wells No Violations On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a digital Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 230 psi. Fluids and gases could not be heard transferring from the wellhead to the flow line. Indicating the well was shut in. I was unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.

Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. 4- Producing wells utilizing natural reservoir pressure.

0760485684 2016-06-27 Production Wells No Violations This well was in production but due to the well drilled and fracked near this well it has caused it to sand off. They have been trying for several weeks to get the sand out of it. The well does not want to flow back so they tried to pump down this well with no success.

1389203903 2014-01-08 Casing String Witnessed No Violations I am on site today to witness the running and setting of the 24 inch diameter casing string. This is a Mine String Casing cement job with H & P Drilling having drilled to 249 ft and will set 247 ft of casing. Halliburton Cementing c/w 190 sks of class A cement. There are two cement baskets one below and 10 ft above the Mine Void . The casing was cemented then set on bottom after the cement was pumped. Then H &P Drilling dumped gravel and water down the back side of the Mine String to open the baskets and then it was cemented to surface. Inspector Chris Grimm was on site to witness the operation.

H & P Drilling Rig # 287 has drilled a 17.5 inch diameter hole to 2120 ft and set 2114 ft of casing. Halliburton Cementing Service c/w 1000 sks of BondCem cement @ 15.8 lb and 2.28 yield
1402369201 2016-09-13 Production Wells No Violations I am on site today accompanied by Supervisor Mike Moreland to perform a site overview and check on the status of the well as a follow up from my inspection last week where I discovered the well was not in production. On the day of my inspection the digital gauge on the top of the production tubing read 245 psi, however the flow line control valve was placed in the closed position and fluid/gas transfer could not be heard going into the flow line. While on site we met with the well tender for the pad, who informed us that the well was sanded off whenever Hess Ohio was preforming the frack job on permitted well number 34-67-2-1419-0000. He stated that to the best of his knowledge Hess Ohio will not be preforming service work on the well until gas and oil prices begin to rise.

1506063589 2014-01-08 Drill / Deepen / Reopen No Violations On the day of my inspection I found that H&P Drilling Rig 287 was at a depth of 247, preparing to run and cement 24 casing as a mine string. The permit was posted on location and I found no leaks or spills.
1513026160 2017-12-11 Production Wells No Violations On the day of my inspection I found this pad containing 4 producing wellheads. Wells heads are on a separate pad from production equipment. Production equipment includes four 400 barrel production tanks, four GPUs,one compressor, and flare. Well is showing 0 psi on the surface casing and 0 psi on intermediate casing. Production records indicate this well has not produced since 3rd quarter 2016.
