0706766982 2015-08-14 Final Restoration No Violations This well has been plugged to surface on 9/02/2014 and Nick Berelsman called to advise that the permit number is now attached to the top of the well casing. It has been cut off below ground level and the tin horn has been pulled and filled to surface. Meets requirements for restoration final.
1161267781 2014-08-04 Drill / Deepen / Reopen No Violations On the day of my inspection I found Sidewinder Rig # 60. attempting to recover 450 feet of drill pipe and the drill assembly. The tool joint broke when completing the drilling process.
1405129720 2014-07-11 Casing String Witnessed No Violations Upon arrival on location I found Sidewinder Rig 60 installing 13 3/8 inch diameter casing to the depth of 320 feet CalFrac will cement to surface. It was determined that Gulfport was not following the casing program approved by the permitting section. After discussions with the Divison Management, it was determined that Gulfports proposed casing program met all well construction requirements and approval was granted to continue with installation of the 13 3/8 inch casing. Gulfport is to contact the permitting section on Monday July 14, 2014 for a revised drilling permit.

They have encountered some lost circulation problems. They are preparing to mix more lost circulation material in there drilling mud.
1406554180 2014-07-28 Drill / Deepen / Reopen No Violations On the day of my inspection I found Sidewinder Rig # 60 at a depth of 15002 feet and a 8 1/2 inch hole. The rig will TD today at a depth of 15330 feet and will set casing at a depth od 15316 feet.
1407382425 2014-08-06 Plug / Plug Back No Violations On the day of my inspection I found Sidewinder Rig 60 preparing to plug back across the clinton formation. A 450 foot drill pipe and drill assembly was lost in the hole at 6686 feet. Several attempts were make to retrieve the tools, the attempts were unsuccessful. It was decided to plug back across the clinton formation. The bottom of the plug was set at 6692 feet. A 600 foot plug was set to seal across the formation and to use as kickoff point to drill around the drill assembly, and reenter the existing well bore.
1408449791 2014-08-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found Sidewinder Rig # 60 has successfully reentered the original wellbore, at 7153 feet and drilled ahead to 7155 feet. They were tripping out of the hole and going to trip back into the hole with a 8 1/2 inch tricone bit, and clean out the wellbore to the original total depth.

1409509760 2014-08-31 Plug / Plug Back No Violations On the day of my inspection I found Sidewinder Rig 60 preparing to run a inflatable packer. It is a Baker Hughes PIP 7 inch outside diameter to 13 1/4 inch outside diameter. It was set at 7885 feet deep. A washout or other diameter difference would not let us to set packer at 7880 feet. It could be set at 7885 feet. Plug #1 set at 7860 feet to 7360 feet. Circulation was established and cement was pumped.
1684437113 2014-09-02 Plug / Plug Back No Violations On the day of my inspection Gulfport continued to plug and abandon the well. 2 3/8 tubing was run to the depth of 300 feet to cement the top section to the surface. Circulation was established and cement was pumped into the wellbore and brought to the surface. Tubing was laid down and the well was topped off.
1783509341 2014-08-29 Drill / Deepen / Reopen No Violations On the day of my inspection I was accompanied by District Supervisor Brent Bear. We found that Sidewinder Rig 60 was preparing to plug and abandon the well. A cement bond log was run. I left location and will return when they are ready to set a inflatable packer. Upon returning to location Sidewinder Rig 60 was running the inflatable packer to the depth of 7880 feet. An attempt to set the packer failed, pushing the packer down the hole to J off of it. Then will run back in the well with a different packer.
1915519082 2014-08-28 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Sidewinder Rig 60 was continuing to attempt to retrieve drill pipe and bit. It was determined that they had grabbed on the fish and was pulling the pipe out of the wellbore.
2093910010 2016-12-02 Production Wells No Violations I am on site today to perform a routine status check of the well bore and the well location. On the day of my inspection I witnessed the lease load was in good repair, however slight erosion was noticed at the beginning of the lease road and the gates for the location were locked. After inspecting the well bore I discovered no high annular pressures and that the well was in producing order. With the inspection I did not discover any indications of pollution or contamination.
2122160744 2014-09-02 Plug / Plug Back No Violations On the date of inspection I accompanied ODNR Inspector Randy Akins to witness the top plug of the well. Refer to Randy Akins report for further details.
