0111652539 2015-02-17 Production Wells No Violations I am on site today with Hal Miller of Gulfport Energy. I am meeting today to review the clean up of the condensate spill which occurred on Saturday 2/14/2015 I found that a 1 inch diameter line connected to the high pressure dump line has been leaking condensate. It is not known for what period of time the line has been leaking. Hall and Associates is on site to test the contamination being removed with a PID meter. They will also collect samples for the lab to verify that the contamination has been removed. The area excavated at this time is 20 ft. x 12 ft. x 5 ft. deep. The contamination being removed is being stored in a roll off container until it can be transported to a landfill.
0299462775 2014-06-29 Casing String Witnessed No Violations Upon arrival on location I found that Sidewinder Rig 60 had installed 7345 feet of 9 5/8 inch diameter casing and CalFrac would be doing the cementing. Established circulation. Had good circulation through out the job. Gel did circulate to surface but did not see any cement returns.

0390035161 2016-01-05 Preliminary Restoration No Violations On the day of my inspection accompanied by Inspector Doug Bennett, I witnessed the well was equipped with a Cactus Wellhead and a production tree. Production equipment for the well included a sand trap, line heater, vertical separator, compressor unit, vapor recovery system, gas combustor, secondary flare stack and 8-400 bbl. tanks inside containment that are shared with 34067211830000 Puskarich 3-18H. The site passed preliminary restoration in accordance with ORC 1509.072 A.
0452494067 2016-12-02 Production Wells No Violations I am on site today to perform a routine status check of the well bore and the well location. On the day of my inspection I witnessed the lease load was in good repair, however slight erosion was noticed at the beginning of the lease road and the gates for the location were locked. After inspecting the well bore I discovered no high annular pressures and that the well was in producing order. With the inspection I did not discover any indications of pollution or contamination.
0965181396 2015-03-03 Production Wells No Violations I am on site today to witness the fill and grade of the hole excavated to remediate the condensate leak. The lab results show the area is clean and can be back filled and the equipment installed so production of the wells can continue.
1042979451 2015-02-24 Production Wells No Violations The reason for my inspection is to check the status of the clean up. The excavation of the contamination has concluded and today I found that dig site open waiting on lab results before it can be filled and graded.
1302660308 2015-02-17 Production Wells No Violations I am on site today with Hal Miller of Gulfport to review the clean up of this site. Saturday I was notified that there was a condensate spill and that Hall and Associates was on site to collect PID readings and to collect samples for the lab. The area is below the high pressure line connected to the dump valve. It has leaked an unknown amount of condensate which is being excavated and removed to roll off containers. The area excavated is 20ft x 12ft x 5 ft. deep. Excavation is to continue today with a Minnie Hoe.
1403052570 2014-06-17 Casing String Witnessed No Violations Upon arrival on location I found Sidewinder Rig # 60 preparing to install 18 5/8 inch diameter casing. CalFrac will be cementing the casing back to surface.
1424268940 2015-02-18 Production Wells No Violations I am on site today to check on the clean up of the condensate spill from 02/14/2015 and found that the digging has ended. I met with Hal Miller and he advised that Hall & Associates screened the dig with a PID meter to find the end of the condensate contamination. They also collected confirmatory samples and sent them to the lab to verify that they have removed all contamination. The dig will remain open until the lab sends the results back to confirm the all clean before it is backfilled.
