0701941833 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap and a gas production unit. Production equipment that was shared with permitted wells 34067212010000, 34067212020000, 34067212030000, 34067212040000, 34067212050000, 34067213240000, and 34067213250000 included a vapor recovery system, a gas compressor, a flare stack and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
0947908169 2016-02-19 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. With the exception of permitted well no. 34067212030000, which is utilizing a plunger lift system to produce. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

1 Horizontal heater-treater that was disconnected 1 combustor 1 low pressure separator/ Vapor recovery system 1 compressor There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1376358473 2013-08-12 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 20 inch diameter casing string. Precision Drilling drilled a 26inch diameter hole 20ft deep and then drilled a 24inch diameter hole to 120ft. Allied Cementing Service will cement the casing with 226 sks of class A cement and had bbls of good returns. The job ran behind because they had to wait on 4 water trucks so they could do both jobs at once. They had hole trouble on the # 5 well and ended up doing them one after the other.
