Production equipment for the well included a sand trap, line heater, vertical separator, compressor unit, vapor recovery system, gas combustor, secondary flare stack 8-400 bbl. tanks inside containment that are shared with 34067212090000 Puskarich 1-18H. The site passed preliminary restoration in accordance with ORC 1509.072 A.
0445888115 2014-09-24 Drill / Deepen / Reopen No Violations On the day of my inspection I found Sidewinder 60 pulling drill pipe out of the wellbore. They have TD the well at a depth of 15240 feet. They will be running 5 1/2 inch casing to complete the well.

0964943456 2014-09-09 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Gulfport company man Brett Hewitt that the Sidewinder 60 rig was at a depth of 2026 feet and had set 13.375 inch Intermediate casing the previous night. The wellhead was currently being welded on to commence drilling. It was reported to me that 20bbl of cement returned to surface on the Intermediate job.
1091917496 2016-12-02 Production Wells No Violations I am on site today to perform a routine status check of the well bore and the well location. On the day of my inspection I witnessed the lease load was in good repair, however slight erosion was noticed at the beginning of the lease road and the gates for the location were locked. After inspecting the well bore I discovered no high annular pressures and that the well was in producing order. With the inspection I did not discover any indications of pollution or contamination.

Franks Casing Crew set 18.625 inch 87.5lbs/ft casing to a depth of 329 feet. I witnessed Cal Frac pump 456sk of Class A Cement weighing 15.6ppg with a yield of 1.21. Additives were 3% CaCl and 0.25lbs/sk of Celloflake. They displaced cement with 93bbl of fresh water. I witnessed 14bbl of cement return to surface.
