0888392790 2013-08-07 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling depths and status of Nomac Drilling Rig # 70 - Met with Company Man Tim Allen and went over drilling information for the # 1- H well. They have reached total depth at 16,330 ft and are out of the hole with drillpipe and are ready to run 5-1/2 inch diameter casing and should cement Thursday morning. Fill out cement ticket and bring up to date while on site.
0986017106 2013-06-25 Drill / Deepen / Reopen No Violations The total depth of the 26 conductor hole is 120. Ran 120 of 20 94 lb/ft casing to 120 and cemented with 230 sacks of class A cement. See the casing ticket for details.
1556931181 2013-10-03 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A.
2026773940 2016-02-24 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are utilizing a plunger lift system. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Horizontal heater-treater 1 combustor 1 compressor 1 Low pressure separator tower/Vapor recovery system unit. The site passed preliminary restoration in accordance with ORC 1509.072 A.
