1007133844 2015-01-20 Production Wells No Violations I am on site today after I noticed equipment entering the well pad. I thought that they may be starting to fill in the hole which had been excavated and found out they are not working on that today.
1034766832 2014-12-03 Production Wells No Violations On the date of inspection I met with Brad Burwell of Gulfport Energy and Kris Andersen of ALL Consulting to receive updates on the condensate cleanup. At the time of my inspection there was approximately a 50 feet long section that had been excavated along the pipes where contamination was reported. They were currently removing gravel and some fill around the pipes with plans to excavate deeper under the pipes in the following days. The entire excavation was approximately 30 feet by 50 feet with the deepest excavtion approximately 3 feet while I was on site. A number of had auger holes had been dug along the piping in order to delineate the length of contaminated soil along the pipes. A Hull and Associates representative was on site performing Photoionization Detector (PID) readings of the contaiminated soil and gravel being removed. According to Kris and Brad, they believe they have reached the end of contamination at the west end and were currently excavating their way east along the pipes. After the soils have been removed around the piping, they will begin excavating underneath the pipes to determine how deep the contamination is present.

1224220192 2016-12-02 Production Wells No Violations I am on site today to perform a routine status check of the well bore and the well location. On the day of my inspection I witnessed the lease load was in good repair and the gates for the location were locked. After inspecting the well bore I discovered no high annular pressures and that the well was in producing order. With the inspection of the location I discovered that Gulfport is utilizing the site to hold cinders for their lease roads. Also with the inspection I did not discover any indications of pollution or contamination.
1365396072 2013-04-07 Drill / Deepen / Reopen No Violations I am on site tonight to witness the running and the cementing of the 26 inch diameter casing string. Redstone Drilling drilled a 32 inch diameter hole to 140 ft and set 140 ft of casing. Stingray Cementing Service c/w 290 sks of class A cement and had 17 bbls of cement returns. Had to wait on water truck, then when it arrived it was empty and then had to wait for it to load.
1366148274 2013-04-16 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 16 inch diameter casing string. Yost Drilling Rig # 7 drilled a 20 inch diameter hole to 380 ft and set 354 ft of casing. Stingray Cementing Serive c/w 370 sks and had 25 bbls of good cment returns
1382973375 2013-10-27 Drill / Deepen / Reopen No Violations Was notified that while cementing of the production string cement locked up and that they were going to squeeze cement on the backside. I requested that before the well was to be completed that a cement bond log would have to be run and a copy supplied to the state. Upon arrival to the site at 6:30am I witnessed that O-Tex was squeezing at a rate of 1bbl per minute. I did request to Company man Greg Armstead that a cement bond log would have to be run and he stated that Gulfport was aware and it would be done before well would be completed.

1416958961 2014-11-24 Production Wells No Violations I received a call from Brad Burwell on 11/24/2014 to advise me that a spill occurred at 5 PM and that 4 to 5 gallons of condensate are outside of the lined dike used to contain the production tanks. He advised that a super sucker vacuum truck was on site and that the spill was being vacuumed up as we speak. I advised Brad to contact me if after digging that they discovered more condensate than first advised. He called today to say that there is more than the 5 gallons first reported and he could not tell be how much. I went to the pad and found that they have dug up a section next to the stand up separator and near the line heater area. They are now 50 ft. x 60 ft. x 2 to 3 ft. deep. Hull Consulting is on site with a PID meter to monitor the clean up. The clean up is to continue until the contamination has been removed. Today there are no roll off containers on site and the soil in being stored on a plastic liner. I advised that the dig site be covered if there is rain and before the clean up is halted for the night. The delineation of the contamination along with the source has not been determined.
1417454251 2014-12-01 Production Wells No Violations I met on site today with Brad Burwell to review the clean up of the condensate spill. The Vertical Heater Treater has been moved so that the area below it can be excavated. The area now is 50ft x 60ft x 4 ft. deep. They hit bedrock sandstone at that depth. In the past that bedrock was fractured and the condensate could have under pressure been forced into that area. Digging continues along the production lines and back toward the south east corner. There are 8 roll off containers on site full of contaminated soil along with a lined dirt pile which is 20 ft. x 25 ft. x 3 ft. which needs to be placed into containers to be removed to a landfill.
1417796027 2014-12-05 Production Wells No Violations I am on site today with Brad Burwell and Kris Anderson. I discovered that they are down to around 7 ft. deep with the excavation of the contaminated soil and sandstone rock. The digging continues deeper to try and find the full extent of the condensate leak. Today the dig site is 70 ft. long 55 to 60 ft. wide and will go past the 7 ft. in depth. More production equipment has been removed and still more will need to be removed in order to excavate the known area where there is contamination. The production line may need to come out to allow access to the material on each side which is testing as contaminated with condensate. I contacted Rick Trippel and Brent Bear to update them on my findings.
1418651216 2014-12-15 Production Wells No Violations I am on site today checking on the clean up status. I found that digging may be complete. They have completed a grid to test the entire clean up area. They are waiting for the results to indicate that they have removed all of the contamination before filling the dig site. The dig site now is 70 ft. x 55 ft. x 9 ft. deep. Contaminated soil continues to be trucked off to the landfill with about two trucks worth of dirt remaining on site.

1607170859 2014-12-30 Production Wells No Violations This operation remains shut down. The area excavated to remove the condensate remains unfilled. They are waiting for the lab samples to show that all of the contamination has been removed before it can be refilled. Talked to Brad Burwell from Gulfport Energy and he advised me that the samples should be back from the lab next day or so.
