0172440643 2017-08-15 Preliminary Restoration No Violations On the day of my inspection I found the Ericson pad to contain two properly identified wellheads, two 500 barrel steel brine tanks with identification contained in a metal dike, no oil tanks, and two sand seperators. No annualar pressure on the 9 5/8 inch diameter casing or on the 13 3/8 inch diameter casing. Pressure on well was approximately 700 psi. Well is equipped with a christmas tree style wellhead and a plunger lift system. Site passes preliminary restoration according to ORC 1509.072A.

2054635965 2013-06-20 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. Nomac Drilling Rig # 70 drilled a 17.5 inch diameter hole to 533 ft and set 523 ft of casing. Shclumberger Cementing Service c/w 458 sks of class A @ 15.6 lbs and a yield of 1.18 and had good cement returns.
