0800535231 2016-02-19 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 6 Producing wells 6 Sand separators

2 Horizontal heater-treater that was disconnected 1 combustor 1 low pressure separator/ Vapor recovery system 1 compressor 2 NGL tanks that were disconnected at the time of my inspection There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1006828140 2013-10-16 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Express Drilling drilled a 26 inch diameter hole to 120 ft and set 120 ft of casing. Allied Cementing Service c/w 250 sks of class A. During the cement job after 15 bbls were mixed they lost circulation and had to pick up the casing and work it to get circulation again. It took almost 1 hour to work it up and down and the cement which was in the casing had to be circulated out before cementing could start again. There were only slight returns and I will check for the top of the cement Thursday when more wells are scheduled.
1391554125 2014-02-04 Casing String Witnessed No Violations On the day of my inspection Nomac Drilling rig 70 ran 13.375 surface casing to 560 in 17.5 hole. O-Tex Pumping attempted to break circulation with fresh water. They pumped 190 barrels without establishing circulation. O-Tex mixed a gel sweep and pumped it down the hole. Circulation was established and O-Tex went straight into cement. Circulation was lost again while mixing and pumping cement. O-Tex slowed the pump rate down and re-established circulation. Circulation was maintained through displacement. There were approximately 16 barrels of cement returns. The cement fell approximately 15 after landing the plug.
1392046979 2014-02-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Drilling Rig 70 was drilling the 8.75 production hole on fluid, at a measured depth of 8500. O-Tex Pumping cemented 9.625 intermediate casing to surface on 2/6/14. The permit was posted and I found no leaks or spills around the location.
1392750436 2014-02-18 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Nomac Drilling rig 70 had set new 5.5 production casing, cemented by O-Tex Pumping.
1686590330 2017-09-22 Preliminary Restoration No Violations On the day of my inspection I found the NM Harrison pad to contain six properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface casing pressure and 100 psi on intermediate. Production equipment on the pad includes six vertical seperators with gas production units, one heater treater, five 400 barrel steel condensate tanks, three 400 barrel steel produced water tanks, vapor recovery system and a combustor. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
