0370690211 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are utilizing a plunger lift system. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 6 Producing wells 6 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 1 compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.

1359508836 2013-01-29 Drill / Deepen / Reopen No Violations I am on site this evening to witness the running and the cementing of the 13 3/8diameter casing string. The 17.5 well was drilled to 610 ft. Nomac Drilling rig # 71 set 600 ft of casing and Baker Hughes Cement Service circulated cement to surface. While running the casing the landing joint was cross threaded and after 4 hours it was corrected.
