1343443086 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are utilizing a plunger lift system. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Horizontal heater-treater 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 1 compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.

Production equipment includes three sand traps, three GPUs, three 400 barrel steel produced water tanks, five 400 barrel steel condensate tanks, a compressor, and a combustor. Tanks are in their own steel containment. Surface annular pressure is 0 and intermediate is 390. Production records are current in RBDMS.
